摘  要:目的探讨癌胚抗原(carcinoembryonic antigen,CEA)、甲胎蛋白(alpha-fetoprotein,AFP)、糖类抗原19-9(carbohydrate antigen 19-9,CA19-9)及糖类抗原72-4(carbohydrate antigen 72-4,CA72-4)联合检测在卵巢良恶性肿瘤诊断中的价值。方法选取2020年6月至2022年6月赣州市妇幼保健院收治的80例卵巢肿瘤患者作为研究对象,按照肿瘤类型分为卵巢恶性肿瘤组(n=42)与卵巢良性肿瘤组(n=38);另选取同时期在本院健康体检人员80名作为对照组。所有研究对象均检测血清AFP、CEA、CA72-4、CA19-9水平,比较3组肿瘤标志物水平及阳性率,并分析肿瘤标志物单一检测和联合检测对卵巢癌的诊断效能。结果3组CEA、AFP、CA19-9、CA72-4水平比较差异有统计学意义(P<0.05);卵巢恶性肿瘤组CEA、AFP、CA19-9、CA72-4水平均高于卵巢良性肿瘤组、对照组,卵巢良性肿瘤组高于对照组,差异有统计学意义(P<0.05)。3组CEA、AFP、CA19-9、CA72-4阳性率比较差异有统计学意义(P<0.05);卵巢恶性肿瘤组CEA、AFP、CA19-9、CA72-4阳性率均高于卵巢良性肿瘤组、对照组,差异有统计学意义(P<0.05)。联合检测的特异度为100.00%,曲线下面积(area under curve,AUC)为0.993,高于CEA、AFP、CA19-9、CA72-4单一检测。结论CEA、AFP、CA19-9及CA72-4联合检测在卵巢良恶性肿瘤诊断具有较高的特异度,可提高卵巢恶性肿瘤诊断准确率,值得临床推广应用。Objective To explore the value of combined detection of carcinoembryonic antigen(CEA),alpha-fetoprotein(AFP),carbohydrate antigen 19-9(CA19-9)and carbohydrate antigen 72-4(CA72-4)in the diagnosis of benign and malignant ovarian tumors.Methods 80 patients with ovarian tumors admitted to Ganzhou Women and Children's Health Care Hospital from June 2020 to June 2022 were selected as the study research subjects,and they were divided into the ovarian malignant tumor group(n=42)and the ovarian benign tumor group(n=38)according to tumor type;and 80 healthy physical examination personnel at the same period in our hospital were selected as the control group.Serum AFP,CEA,CA72-4 and CA19-9 indicators were detected in all subjects,the levels and positive rates of tumor markers were compared among the three groups,and the diagnostic efficacy of single and combined detection of tumor markers for ovarian cancer was analyzed.Results There were significant differences in CEA,AFP,CA19-9 and CA72-4 levels among the three groups(P<0.05);the levels of CEA,AFP,CA19-9 and CA72-4 in the ovarian malignant tumor group were higher than those in the ovarian benign tumor group and the control group,the ovarian benign tumor group was higher than the control group,and the differences were statistically significant(P<0.05).There were significant differences in CEA,AFP,CA19-9 and CA72-4 positive rates among the three groups(P<0.05);the positive rates of CEA,AFP,CA19-9 and CA72-4 in the ovarian malignant tumor group were higher than those in the ovarian benign tumor group and the control group,and the differences were statistically significant(P<0.05).The specificity of combined diagnosis was 100.00%,area under curve(AUC)was 0.993,which were higher than the single detection of CEA,AFP,CA19-9 and CA72-4.Conclusion The combined detection of CEA,AFP,CA19-9 and CA72-4 in the diagnosis of ovarian malignant tumors has high specificity,can improve the diagnostic accuracy of ovarian malignant tumorsr,and is worthy of clinical promotion and application.
