外周血CTCs水平对口腔鳞状细胞癌的诊断价值

摘　　要：目的:探讨外周血循环肿瘤细胞(CTCs)水平对口腔鳞状细胞癌(OSCC)的诊断价值。方法:选取2021年5月—2024年2月在枣庄市口腔医院口腔颌面外科就诊的98例OSCC患者为研究组,同期选取37例健康体检者为对照组。通过阴性富集法和靶向聚合酶链式反应(PCR)技术对外周血样本中的CTCs进行检测。采用Pearson相关分析CTCs水平与OSCC临床特征的相关性,采用受试者操作特征(ROC)曲线评估CTCs水平对OSCC的诊断价值。结果:研究组CTCs水平为(6.53±1.68)FU/3mL,高于对照组的(4.09±1.10)FU/3mL,差异有统计学意义(P<0.05)。不同TNM分期、淋巴结转移情况和分化程度OSCC患者外周血CTCs水平比较,差异有统计学意义(P<0.05)。Pearson相关分析显示,外周血CTCs水平与TNM分期、淋巴结转移呈正相关,与肿瘤分化程度呈负相关(P<0.05)。ROC曲线分析显示,外周血CTCs水平诊断OSCC的曲线下面积为0.905,当CTCs为5.07 FU/3mL时,约登指数最大为0.702,敏感度为83.67%,特异度为86.49%。结论:外周血CTCs水平在OSCC的诊断中具有重要的价值,CTCs水平变化可用于监测肿瘤进展情况。Objective:To investigate the diagnostic value of circulating tumor cells(CTCs)in peripheral blood for oral squamous cell carcinoma(OSCC).Methods:A total of 98 patients with OSCC who were treated in the Department of Oral and Maxillofacial Surgery of Zaozhuang Stomatdogical Hospital from May 2021 to February 2024 were selected as the study group,and 37 healthy subjects were selected as the control group.CTCs in peripheral blood samples were detected by negative enrichment and targeted PCR.Pearson correlation coefficient was used to analyze the correlation between CTCs level and clinical features of OSCC,and receiver operating characteristic(ROC)curve was used to evaluate the diagnostic value of CTCs level for OSCC.Results:The level of CTCs in the study group was(6.53±1.68)FU/3 mL,which was higher than(4.09±1.10)FU/3 mL in the control group,and the difference was statistically significant(P<0.05).There was a statistically significant difference in the levels of peripheral blood CTCs among OSCC patients with different TNM stages,lymph node metastasis status,and differentiation degree(P<0.05).Pearson correlation coefficient analysis showed that the level of CTCs in peripheral blood was positively correlated with TNM stage and lymph node metastasis,and negatively correlated with tumor differentiation(P<0.05).ROC curve analysis showed that the area under the curve of peripheral blood CTCs level in the diagnosis of OSCC was 0.905.When the CTCs was 5.07 FU/3 mL,the Youden index was 0.702,the sensitivity was 83.67%,and the specificity was 86.49%.Conclusion:The level of CTCs in peripheral blood has important value in the diagnosis of OSCC,and the change of CTCs level can be used to monitor tumor progression.
