摘 要:目的分析血清标志物性别决定区Y框蛋白2(SOX2)、G抗原7(GAGE7)、细胞角蛋白19片段(CYFRA21-1)水平对非小细胞肺癌(NSCLC)患者的早期诊断效能。方法选取2022年2月至2024年2月新疆维吾尔自治区人民医院收治的69例NSCLC患者为观察组,另选取新疆维吾尔自治区人民医院同期收治的70例肺部良性病变患者为对照组。比较分析两组患者的血清标志物[SOX2、GAGE7、人癌抗原(CAGE)、癌胚抗原(CEA)、神经元特异性烯醇化酶(NSE)、鳞状细胞癌抗原(SCCA)、CYFRA21-1]水平。采用Spearman分析血清标志物水平与NSCLC发生的相关性。采用多因素logistic回归方法分析NSCLC早期诊断的独立影响因素。采用受试者操作特征(ROC)曲线分析血清标志物SOX2、GAGE7、CYFRA21-1水平对NSCLC的早期诊断价值。结果观察组患者的血清SOX2、GAGE7、CAGE、CEA、NSE、SCCA、CYFRA21-1水平均高于对照组,差异均有统计学意义(P<0.05)。Spearman相关性分析结果显示,血清SOX2、GAGE7、CAGE、CEA、NSE、SCCA、CYFRA21-1水平与NSCLC的发生均呈显著正相关(r=0.394、0.351、0.215、0.222、0.228、0.163、0.371,P<0.05)。多因素logistic回归分析结果显示,SOX2、GAGE7、CYFRA21-1水平升高均为NSCLC早期诊断的独立影响因素(P<0.05)。ROC曲线分析结果显示,SOX2、GAGE7、CYFRA21-1三者联合早期诊断NSCLC的曲线下面积为0.862,大于SOX2(曲线下面积为0.727)、GAGE7(曲线下面积为0.703)、CYFRA21-1(曲线下面积为0.714)单一指标。结论与肺部良性病变患者比较,NSCLC患者的血清SOX2、GAGE7、CYFRA21-1水平均显著升高,且SOX2、GAGE7、CYFRA21-1三者联合检测有利于临床早期诊断NSCLC。Objective To analyze the early diagnostic efficacy of serum marker sex determining region Y-box 2(SOX2),G antigen 7(GAGE7)and cytokeratin 19 fragment(CYFRA21-1)in patients with non-small cell lung cancer(NSCLC).Method 69 patients with NSCLC admitted to the People’s Hospital of Xinjiang Uygur Autonomous Region from February 2022 to February 2024 were selected as the observation group,and 70 patients with benign lung lesions admitted to the People’s Hospital of Xinjiang Uygur Autonomous Region during the same period were selected as the control group.Serum markers of SOX2,GAGE7,CAGE,CEA,NSE,SCCA,CYFRA21-1 were compared between the two groups.The correlation between serum marker level and NSCLC was analyzed by Spearman.Multivariate logistic regression was used to analyze the independent factors influencing the early diagnosis of NSCLC.The value of serum markers SOX2,GAGE7 and CYFRA21-1 in the early diagnosis of NSCLC was analyzed by ROC curve.Result The serum levels of SOX2,GAGE7,CAGE,CEA,NSE,SCCA and CYFRA21-1 in observation group were higher than those in control group,with statistical significance(P<0.05).Spearman correlation analysis showed that serum levels of SOX2,GAGE7,CAGE,CEA,NSE,SCCA and CYFRA21-1 were significantly positively correlated with the occurrence of NSCLC(r=0.394,0.351,0.215,0.222,0.228,0.163,0.371,P<0.05).Multivariate logistic regression analysis showed that the increased levels of SOX2,GAGE7 and CYFRA21-1 were independent factors for the early diagnosis of NSCLC(P<0.05).ROC curve analysis results showed that the area under the curve of SOX2,GAGE7 and CYFRA21-1 combined for early diagnosis of NSCLC was 0.862,which was larger than SOX2(with area under the curve of 0.727),GAGE7(with area under the curve of 0.703),CYFRA21-1(with area under the curve of 0.714)single index.Conclusion Compared with patients with benign lung lesions,the serum levels of SOX2,GAGE7 and CYFRA21-1 in patients with NSCLC were significantly higher,and the combined detection of SOX2,GAGE7 and CYFRA21-1 was conducive to t
