摘  要:目的探讨食管癌组织中表皮生长因子受体(EGFR)、血管内皮生长因子-C(VEGF-C)mRNA表达水平与临床病理特征相关性,并分析其对放疗敏感性的预测价值。方法选取2021年12月至2023年12月南阳市中心医院收治的102例食管癌患者为研究对象,经病理组织活检切除癌组织、癌旁组织,比较其EGFR、VEGF-C mRNA表达水平,并分析其与临床病理特征相关性;依据放疗敏感性将患者分为敏感组、抵抗组,比较2组患者癌组织EGFR、VEGF-C mRNA表达水平,分析其对放疗抵抗性的影响;评价放疗前癌组织EGFR、VEGF-C mRNA对放疗抵抗性的预测价值。结果食管癌组织EGFR、VEGF-C mRNA表达水平均高于癌旁组织(t=24.842,P<0.001;t=36.007,P<0.001),与组织分化程度(r=-0.417,P<0.001;r=-0.463,P<0.001)呈负相关,而与TNM分期(r=0.528,P<0.001;r=0.577,P<0.001)、淋巴结转移(r=0.477,P<0.001;r=0.602,P<0.001)呈正相关;敏感组放疗前、后癌组织EGFR(t=9.107,P<0.001;t=12.350,P<0.001)、VEGF-C(t=14.570,P<0.001;t=13.301,P<0.001)mRNA表达水平均低于抵抗组;癌组织EGFR、VEGF-C mRNA均是放疗抵抗性的独立危险因素(OR=1.432,P<0.001;OR=1.467,P<0.001);放疗前癌组织EGFR、VEGF-C mRNA单项及联合预测放疗抵抗性的AUC分别为0.848、0.771、0.967,联合预测AUC高于单项指标预测(Z=2.635,P=0.012;Z=2.781,P=0.010)。结论食管癌组织中EGFR、VEGF-C mRNA表达水平升高,与组织分化程度呈负相关,而与TNM分期、淋巴结转移呈正相关,且EGFR、VEGF-C均是放疗抵抗性的独立危险因素,联合检测其mRNA表达水平对放疗敏感性具有一定预测价值。Objective To investigate the correlation between the mRNA expression levels of epidermal growth factor receptor(EGFR)and vascular endothelial growth factor-C(VEGF-C)in esophageal cancer tissues and their clinicopathological characteristics,and to analyze their predictive value for radiotherapy sensitivity.Methods The 102 patients with esophageal cancer admitted to Nanyang Central Hospital from December 2021 to December 2023 were selected as the research subjects.Pathological tissue biopsy was performed to remove cancer tissue and adjacent tissue,and the expression levels of EGFR and VEGF-C mRNA were compared,and their correlation with clinicopathological characteristics was analyzed.Patients with esophageal cancer were divided into the sensitive group and the resistant group according to their radiotherapy sensitivity.The expression levels of EGFR and VEGF-C in mRNA esophageal cancer tissues of the sensitive group and the resistant group were compared.The impact of EGFR and VEGF-C mRNA in the esophageal cancer tissues on radiotherapy resistance was analyzed.The predictive value of EGFR and VEGF-C mRNA in esophageal cancer tissues for radiotherapy resistance before radiotherapy was evaluated.Results The expression levels of EGFR and VEGF-C mRNA in the esophageal cancer tissues were higher than those in the paracancerous tissues(t=24.842,P<0.001;t=36.007,P<0.001).The expression levels of EGFR and VEGF-C mRNA in the esophageal cancer tissues were negatively correlated with the degree of tissue differentiation(r=-0.417,P<0.001;r=-0.463,P<0.001),while positively correlated with TNM staging(r=0.528,P<0.001;r=0.577,P<0.001)and lymph node metastasis(r=0.477,P<0.001;r=0.602,P<0.001).The expression levels of EGFR(t=9.107,P<0.001;t=12.350,P<0.001)and VEGF-C(t=14.570,P<0.001;t=13.301,P<0.001)mRNA in the esophageal cancer tissues of the sensitive group before and after radiotherapy were higher than those of the resistant group.Both EGFR and VEGF-C mRNA in the esophageal cancer tissues were independent risk factors for radiati
