宫颈环形电切术联合干扰素治疗人乳头瘤病毒感染合并宫颈癌前病变的应用价值分析

摘　　要：目的探讨宫颈环形电切术(LEEP)联合干扰素对人乳头瘤病毒(HPV)感染合并宫颈癌前病变的治疗价值。方法选择2019年11月至2020年11月新乡市中心医院收治的60例HPV感染合并宫颈癌前病变患者,以随机抽样法将患者分为对照组、研究组2组,每组30例。对照组给予单纯LEEP刀治疗,研究组给予LEEP刀^(+)干扰素联合治疗。比较2组临床疗效、T淋巴细胞水平、并发症发生率、不良反应发生率。结果研究组总有效率、HPV转阴率均高于对照组,而3个月内HPV感染复发率低于对照组(χ^(2)=7.937,P=0.005;χ^(2)=4.286,P=0.038;χ^(2)=6.405,P=0.011)。研究组治疗后CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)水平均高于对照组,而CD8^(+)水平低于对照组(t=3.830,P<0001;t=6.328,P<0001;t=5.837,P<0001;t=3.260,P=0.002)。2组并发症、不良反应总发生率比较差异均无统计学意义(χ^(2)=1.456,P=0.228;χ^(2)=3.158,P=0.076)。结论LEEP刀联合干扰素对HPV感染合并宫颈癌前病变安全有效,且能够促进患者HPV转阴,降低HPV感染复发风险。Objective To appraise the clinical value of interferon plus loop electrosurgical excision procedure(LEEP)in the treatment of human papillomavirus(HPV)infection with cervical precancerous changes.Methods The 60 HPV infection patients with cervical precancerous changes treated from November 2019 to November 2020 in Xinxiang Central Hospital were selected and randomly divided into the control group(30 cases)and the research group(30 cases).The control group just took LEEP,the research group took LEEP and interferon.The clinical efficacy,T lymphocyte level,complication incidence and adverse reaction incidence were compared.Results The total effective rate,the HPV negative rate of the study group were higher than those of the control group,but the recurrence rate of HPV infection of the research group was lower than that of the control group(χ^(2)=7.937,P=0.005;χ^(2)=4.286,P=0.038;χ^(2)=6.405,P=0.011).The CD3^(+),CD4^(+)and CD4^(+)/CD8^(+)levels after treatment in research group were higher than those in control group(t=3.830,P<0001;t=6.328,P<0001;t=5.837,P<0001;t=3.260,P=0.002).The statistic results were not significant in the incidence of complication and adverse reaction(χ^(2)=1.456,P=0.228;χ^(2)=3.158,P=0.076).Conclusion LEEP plus interferon has the obvious effect and safety for HPV infection patients with cervical precancerous changes,which can increase the negative conversion rate of human papillomavirus and reduce the risk of recurrent HPV infection.
