摘　　要：目的探讨内镜下染色技术在结肠无蒂锯齿状病变(SSL)冷切除术中的应用价值。方法选取2019年1月至2022年6月在郑州颐和医院行结肠镜检查并发现的结肠SSL患者为研究对象,将SSL以直径大小分为A组(≤5 mm)、B组(>5~10 mm),A组与B组进一步随机分为A1组、A2组、B1组、B2组,A1组与B1组于普通白光内镜下行结肠SSL的冷切除治疗,A2组与B2组使用醋酸染色后行SSL冷切除治疗,比较观察各组患者一般资料、术后切缘阳性率等。结果A1组与A2组冷切除术后切缘阳性率比较差异无统计学意义(χ^(2)=4.698,P=0.056),B1组与B2组冷切除术后切缘阳性率比较差异有统计学意义(χ^(2)=7.974,P=0.006)。结论醋酸染色可降低内镜下结肠SSL(>5~10 mm)冷切除术后切缘阳性风险。Objective To study the application value of chromoendoscopy in the cold resection of colonic sessile serrated lesion(SSL).Methods The colonic SSL detected by colonoscopy in Zhengzhou Yihe Hospital from January 2019 to June 2022 were selected as the study subjects.These colonic SSL were divided into group A(5 mm)and group B(>5~10 mm)by diameter size.Group A and group B were randomly divided into group A1,and group A2,group B1 and group B2.Group A1 and B1 underwent cold resection of colonic SSL under ordinary white light endoscopy.Group A2 and B2 were treated with acetic acid stain followed by cold resection of colonic SSL.General patient datum and postoperative margin positive rates were analyzed.Results The comparison of postoperative margin positive rates after cold resection among groups A1 and A2 showed no statistically significant differences(χ^(2)=4.698,P=0.056).A statistically significant difference was observed in postoperative margin positive rates between group B1 and B2(χ^(2)=7.974,P=0.006).Conclusion Acetic acid stain could reduce the positive rates of postoperative margins after cold resection of colonic SSL(>5~10 mm).
