眼内套环联合无瓣人工晶体固定术治疗无晶体眼疗效观察

摘　　要：目的 探讨眼内套环联合无瓣人工晶体固定术治疗无晶体眼的临床疗效和安全性。方法 回顾性分析32例32眼行人工晶体巩膜固定术患者的临床资料,按手术方式不同分为研究组13例13眼和对照组19例19眼。研究组采用眼内套环联合无瓣人工晶体固定术,对照组采用睫状沟缝线固定悬吊人工晶体植入术。观察术后1周、3个月的视力、眼压、角膜内皮计数、六边形细胞比率和角膜中央厚度、手术时间、人工晶体位置、术后并发症等情况。结果 术后1周研究组视力明显优于对照组(P<0.05),术后3个月2组视力差异无统计学意义(P>0.05);研究组手术时间明显短于对照组(P<0.05);术后眼压、角膜内皮计数、六边形细胞比率和角膜中央厚度组间差异无统计学意义(P>0.05);术后人工晶体位置情况和术后并发症发生率组间差异无统计学意义(P>0.05)。结论 采用眼内套环联合无瓣人工晶体固定术治疗无晶体眼手术时间短、术后恢复快,可明显提高患者术后视力,手术效果确切,值得临床参考应用。Objective To investigate the clinical efficacy and safety of intraocular ring combined with valveless intraocular lens fixation in the treatment of aphakic eyes.Methods The clinical data of 32 patients(32 eyes)who underwent intraocular lens suture fixation were retrospectively analyzed.According to the different surgical methods,they were divided into the study group(13 cases,13 eyes)and the control group(19 cases,19 eyes).The study group was treated with intraocular ring combined with valveless intraocular lens fixation,and the control group was treated with ciliary sulcus suture fixation suspension intraocular lens implantation.The visual acuity,intraocular pressure,corneal endothelial count,hexagonal cell ratio and central corneal thickness,operation time,intraocular lens position and postoperative complications were observed at 1 week and 3 months after operation.Results One week after operation,the visual acuity of the study group was significantly better than that of the control group(P<0.05),while there was no statistically significant difference in visual acuity between the two groups at three months after surgery(P>0.05).The operation time of the study group was significantly shorter than that of the control group(P<0.05).There was no statistically significant difference in postoperative intraocular pressure,corneal endothelial count,hexagonal cells ratio,and central corneal thickness between the groups(P>0.05).There was no significant difference in the position of intraocular lens and the incidence of postoperative complications between the two groups(P>0.05).Conclusion The use of intraocular ring combined with valveless intraocular lens fixation for the treatment of aphakic eyes has a short operation time and rapid postoperative recovery,which can significantly improve the postoperative visual acuity of patients.The surgical effect is accurate and worthy of clinical reference.
