摘 要:目的:评估子宫肌瘤患者的睡眠质量,探讨其与血清激素水平动态波动的相关性,以深入理解激素失衡对睡眠障碍的潜在影响。方法:选取2023年1月至2023年6月陵城区人民医院妇产科收治的子宫肌瘤患者120例作为研究对象,分为睡眠质量较差组及睡眠质量良好组,每组60例。采用匹兹堡睡眠质量指数(PSQI)比较2组患者睡眠质量的差异,通过血清检测监测受试者在月经周期不同阶段的雌二醇(E_(2))、孕酮(P4)、促卵泡激素(FSH)和黄体生成素(LH)水平,并比较患者的雌激素水平与睡眠质量的相关性。结果:睡眠质量较差组患者的E_(2)、P4水平显著低于睡眠质量良好组,而FSH水平显著高于睡眠质量良好组,差异均有统计学意义(均P<0.01)。E_(2)水平与PSQI评分负相关(r=-0.49,P<0.01),FSH水平与PSQI评分正相关(r=0.56,P<0.01),提示E_(2)水平越低、FSH水平越高,睡眠质量越差。结论:子宫肌瘤患者的睡眠质量与激素水平波动密切相关,尤其是E_(2)水平的降低和促卵泡激素水平的升高对睡眠障碍的影响显著。Objective:To evaluate the sleep quality of patients with uterine fibroids and explore its correlation with dynamic fluctuations in serum hormone levels,aiming to gain a deeper understanding of the potential impact of hormone imbalance on sleep disorders.Methods:A total of 120 patients with uterine fibroids admitted to the Obstetrics and Gynecology Department of Lingcheng District People′s Hospital from January 2023 to June 2023 were selected as the research subjects and divided into a poor sleep quality group and a good sleep quality group,with 60 patients in each group.The Pittsburgh Sleep Quality Index(PSQI)was used to compare the differences in sleep quality between two groups.Serum tests were conducted to monitor the levels of estradiol(E_(2)),progesterone(P4),follicle stimulating hormone(FSH),and luteinizing hormone(LH)at different stages of the menstrual cycle,and the correlation between estrogen levels and sleep quality was compared.Results:The E_(2)and P4 levels of patients in the poor sleep quality group were significantly lower than those in the good sleep quality group,while the FSH level was significantly higher than that in the good sleep quality group.The differences between the two groups were statistically significant(P<0.01).E_(2)levels were negatively correlated with PSQI scores(r=-0.49,P<0.01),while FSH levels were positively correlated with PSQI scores(r=0.56,P<0.01),indicating that lower E_(2)levels and higher FSH levels lead to poorer sleep quality.Conclusion:The sleep quality of patients with uterine fibroids is closely related to fluctuations in hormone levels,especially the significant impact of decreased estradiol levels and increased follicle stimulating hormone levels,which significantly impact sleep disorders.
