超声内镜术前诊断胃癌TNM分期及与miRNA-134-3p、血管内皮生长因子表达的关系

摘　　要：目的探讨超声内镜(EUS)术前诊断胃癌TNM分期及与miRNA-134-3p、血管内皮生长因子(VEGF)表达的关系。方法选取90例胃癌患者,术前接受EUS检查,评估EUS检查对胃癌患者TNM分期的诊断价值。取所有患者的胃癌组织及相应癌旁组织,采用逆转录聚合酶链反应(RT-PCR)检测miRNA-134-3p、VEGF mRNA的相对表达量,比较不同EUS术前诊断TNM分期的胃癌患者胃癌组织中miRNA-134-3p、VEGF mRNA的相对表达量。miRNA-134-3p、VEGF的表达与EUS检查诊断胃癌患者T、N、M分期的相关性采用Point-biserial相关性分析。结果EUS检查术前诊断胃癌患者T分期的准确度为78.89%,诊断N分期的准确度为36.67%,诊断M分期的准确度为54.44%。胃癌组织中miRNA-134-3p的相对表达量明显低于癌旁组织,VEGF mRNA的相对表达量明显高于癌旁组织,差异均有统计学意义(P﹤0.01)。随着EUS术前诊断T、N、M分期的增加,胃癌患者胃癌组织中miRNA-134-3p的相对表达量逐渐降低,VEGF mRNA的相对表达量逐渐升高,差异均有统计学意义(P﹤0.01)。miRNA-134-3p的表达与胃癌患者T、N、M分期均呈明显负相关(r=-0.504、-0.517、-0.533,P﹤0.01),VEGF的表达与T、N、M分期均呈明显正相关(r=0.511、0.519、0.528,P﹤0.01)。结论EUS检查对胃癌患者TNM分期的诊断有一定的价值,miRNA-134-3p的表达与胃癌患者TNM分期呈负相关,VEGF的表达与TNM分期呈正相关。Objective To explore the relationship between preoperative diagnosis of gastric cancer TNM staging by endoscopic ultrasonography(EUS)and tits relationship with miRNA-134-3p and vascular endothelial growth factor(VEGF)expression.Method A total of 90 patients with gastric cancer were selected,all of them underwent EUS examination before surgery,and the diagnostic value of EUS examination for TNM staging of gastric cancer patients was evaluated.The gastric cancer tissues and corresponding adjacent tissues were collected,and the relative expression of miRNA-134-3p and VEGF mRNA were detected by reverse transcription-polymerase chain reaction(RT-PCR).The relative expression of miRNA-134-3p and VEGF mRNA in gastric cancer tissues of gastric cancer patients with TNM staging were diagnosed by EUS examination before surgery were compared.The correlation between the expression of miRNA-134-3p,VEGF and the diagnosis of T,N,and M staging in gastric cancer patients by preperative EUS examination was analyzed by Point-biserial correlation analysis.Result The accuracy of preoperative EUS examination in diagnosing T staging of gastric cancer patients was 78.89%,the accuracy in diagnosing N staging was 36.67%,and the accuracy in diagnosing M staging was 54.44%.The relative expression of miRNA-134-3p in gastric cancer tissues was significantly lower than that in adjacent tissues,and the relative expression level of VEGF mRNA was significantly higher than that in adjacent tissues,the differences were statistically significant(P<0.01).With the increase of T,N,and M staging by preoperative EUS examination,the relative expression of miRNA-134-3p in gastric cancer tissue gradually decreased,while the relative expression of VEGF mRNA gradually increased,and the differences were statistically significant(P<0.01).The expression of miRNA-134-3p was significantly negatively correlated with T,N,and M staging in gastric cancer patients(r=-0.504,-0.517,-0.533,P<0.01),while the expression of VEGF was significantly positively correlated with T,N
