常规超声与超声造影对早期微小肝癌的诊断价值

摘　　要：目的探讨常规超声与超声造影(CEUS)对早期微小肝癌的诊断价值。方法选取87例肝微小病灶患者,均接受常规超声、CEUS检查,以穿刺病理活检结果为金标准,评估常规超声、CEUS对早期微小肝癌的诊断价值,常规超声、CEUS诊断微小肝癌的结果与穿刺活检结果的一致性采用Kappa检验。结果87例肝微小病灶患者均完成穿刺病理活检,其中42例确诊为早期微小肝癌,肝脏良性病变45例。常规超声检查诊断早期微小肝癌的灵敏度为80.95%,特异度为88.89%,准确度为85.06%,阳性预测值为87.18%,阴性预测值为83.33%,其与穿刺病理活检结果的一致性一般(Kappa=0.700);CEUS诊断早期微小肝癌灵敏度为95.24%,特异度为93.33%,准确度为94.25%,阳性预测值为93.02%,阴性预测值为95.45%,其与穿刺病理活检结果的一致性极好(Kappa=0.885);CEUS诊断早期微小肝癌的灵敏度、准确度均高于常规超声检查,差异均有统计学意义(P﹤0.05)。结论CEUS诊断早期微小肝癌的灵敏度、准确度均高于常规超声检查,且与穿刺病理活检结果的一致性极好。Objective To analyze the diagnostic value of conventional ultrasound and contrast-enhanced ultrasound(CEUS)in early micro hepatocellular carcinoma.Method A total of 87 patients with micro liver lesions were selected and underwent conventional ultrasound and CEUS examinations.Puncture pathological biopsy result was used as the gold standard to evaluate the diagnostic value of conventional ultrasound and CEUS for early micro hepatocellular carcinoma.The consistency between the results of conventional ultrasound and CEUS and puncture biopsy diagnosis in diagnosis of micro hepatocellular carcinoma was determined by Kappa test.Result Puncture pathological biopsy result showed that among 87 patients with micro liver lesions,42 cases were early micro hepatocellular carcinoma,and 45 cases were benign liver lesions.The sensitivity,specificity,accuracy,positive predictive value,and negative predictive value of conventional ultrasound examination in diagnosis of early micro hepatocellular carcinoma were 80.95%,88.89%,85.06%,87.18%,and 83.33%,respectively,the consistency between conventional ultrasound examination and puncture pathological biopsy was generally good(Kappa=0.700).The sensitivity,specificity,accuracy,positive predictive value,and negative predictive value of CEUS in diagnosis of early micro hepatocellular carcinoma were 95.24%,93.33%,94.25%,93.02%,and 95.45%,respectively,the consistency between CEUS and puncture pathological biopsy was excellent(Kappa=0.885).The sensitivity and accuracy of CEUS in diagnosing early micro hepatocellular carcinoma were higher than those of conventional ultrasound examination,and the differences were statistically significant(P<0.05).Conclusion The sensitivity and accuracy of CEUS in diagnosing early micro hepatocellular carcinoma are higher than those of conventional ultrasound examination,and the consistency with the results of puncture pathological biopsy is excellent.
