摘 要:目的 分析急性髓系白血病(AML)患者接受中大剂量阿糖胞苷(Ara-C)巩固化疗后发生医院感染的影响因素。方法 选取60例AML患者,均接受中大剂量Ara-C巩固化疗,统计所有患者医院感染的发生情况,AML患者接受中大剂量Ara-C治疗后发生医院感染的影响因素采用Logistic回归分析。结果 60例AML患者接受中大剂量Ara-C治疗后,发生医院感染40例,占比66.67%(40/60),未发生医院感染20例。单因素分析结果显示,感染与未感染患者预防性使用抗生素情况、使用层流床情况、低蛋白血症情况及粒细胞缺乏持续时间比较,差异均有统计学意义(P﹤0.05)。多因素Logistic回归分析结果显示,未使用层流床、未预防性使用抗生素、粒细胞缺乏持续时间≥7天、有低蛋白血症均是AML患者接受中大剂量Ara-C治疗后发生医院感染的独立危险因素(P﹤0.05)。结论 未使用层流床、未预防性使用抗生素、粒细胞缺乏持续时间≥7天、有低蛋白血症均是AML患者接受中大剂量Ara-C巩固化疗后发生医院感染的独立危险因素。Objective To analysis the influencing factors of hospital infection in patients with acute myeloid leukemia(AML)receiving medium and high doses of cytosine arabinoside(Ara-C)consolidation chemotherapy.Method A total of 60 AML patients who received medium to high doses of Ara-C consolidation chemotherapy were selected,and record the incidence of hospital infections in all patients.Logistics regression analysis was used to analysis the influencing factors of hospital infections in AML patients after receiving medium to high doses of Ara-C treatment.Result After receiving medium to high doses of Ara-C treatment,40 cases out of 60 AML patients developed hospital acquired infections,accounting for 66.67%(40/60),while 20 cases did not.Univariate analysis results showed that there were statistically significant differences in the prophylactic use of antibiotics,laminar flow bed use,hypoalbuminemia,and duration of granulocyte deficiency between infected and uninfected patients(P<0.05).Multivariate Logistic regression analysis results showed that no use of laminar flow bed,no prophylactic use of antibiotics,duration of granulocyte deficiency≥7 days,and hypoalbuminemia were all independent risk factors for hospital acquired infections in AML patients treated with medium to high doses of Ara-C(P<0.05).Conclusion No use laminar flow beds,no prophylactic use of antibiotics,duration of granulocyte deficiency≥7 days,and hypoalbuminemia are all independent risk factors for hospital acquired infections in AML patients after receiving medium to high doses of Ara-C consolidation chemotherapy.
