摘　　要：目的 探讨彩色多普勒超声血流参数[搏动指数(PI)、舒张末期流速(EDV)、阻力指数(RI)、收缩期峰值流速(PSV)]与血清卵泡抑素(FS)单独及联合检查对甲状腺癌的诊断价值。方法 选取106例甲状腺癌患者作为观察组,89例良性甲状腺肿瘤患者作为对照组,比较两组患者的彩色多普勒超声血流参数及血清FS水平。采用受试者工作特征(ROC)曲线及曲线下面积(AUC)分析彩色多普勒超声血流参数及血清FS单独及联合检查对甲状腺癌的诊断价值。结果 观察组患者PI、RI低于对照组,EDV、PSV、FS水平高于对照组,差异均有统计学意义(P﹤0.05)。ROC曲线显示,彩色多普勒超声血流参数与血清FS联合检查诊断甲状腺癌的AUC、特异度最高,均高于各指标单独检查。结论 彩色多普勒超声血流参数与血清FS联合检查诊断甲状腺癌的价值较高。Objective To explore the diagnostic value of color Doppler ultrasound blood flow parameters[pulsatility index(PI),end-diastolic velocity(EDV),resistance index(RI),peak systolic velocity(PSV)]and serum follistatin(FS)single and combined examination for thyroid cancer.Method A total of 106 patients with thyroid cancer were selected as the observation group,and 89 patients with benign thyroid tumors were selected as the control group.The blood flow parameters of color Doppler ultrasound and serum FS levels of the two groups were compared.The receiver operating characteristic(ROC)curve and area under the curve(AUC)were used to analyze the diagnostic value of color Doppler ultrasound blood flow parameters and serum FS single and combined examination for thyroid cancer.Result The PI and RI of observation group were lower than those of control group,the levels of EDV,PSV,and FS were higher than those of the control group,and the differences were statistically significant(P<0.05).The ROC curve showed that the AUC and specificity of color Doppler ultrasound blood flow parameters and serum FS combined examination in diagnosing thyroid cancer were highest,and were higher than those of each index single examination.Conclusion The combined examination of color Doppler ultrasound blood flow parameters and serum FS has a high diagnostic value for thyroid cancer.
