摘 要:目的 探讨超声造影(CEUS)对乳腺癌分子分型的诊断价值。方法 选取108例乳腺癌患者,均接受CEUS检查,以免疫组化结果为金标准,评估CEUS对乳腺癌分子分型的诊断价值,其与免疫组化结果的一致性采用Kappa检验。结果 免疫组化结果显示,108例乳腺癌患者中,Luminal A型42例,Luminal B型30例,人表皮生长因子受体2(HER2)过表达型20例,三阴性乳腺癌(TNBC)16例。不同分子分型乳腺癌患者临床分期、淋巴结状态比较,差异均有统计学意义(P﹤0.05)。不同分子分型乳腺癌患者增强模式、增强幅度、增强后边界清晰情况和内部充盈缺损情况比较,差异均有统计学意义(P﹤0.05)。CEUS诊断乳腺癌分子分型的准确度为87.04%(94/108),其与病理检查结果的一致性较高(Kappa=0.820)。结论 CEUS诊断乳腺癌分子分型的准确度较高,其与病理检查结果的一致性较高,为临床手术、辅助化疗等治疗提供参考依据。Objective To explore the diagnostic value of contrast-enhanced ultrasound(CEUS)in molecular subtypes of breast cancer.Method A total of 108 patients with breast cancer were selected,all of them received CEUS examination.Using immunohistochemical results as the gold standard,the diagnostic value of CEUS for molecular subtypes of breast cancer was evaluated.The consistency between CEUS and immunohistochemical results was tested by Kappa test.Result Immunohistochemical results showed that among 108 cases of breast cancer,42 cases were Luminal A type,30 cases were Luminal B type,20 cases were human epidermal growth factor receptor 2(HER2)overexpression type,and 16 cases were triple negative breast cancer(TNBC).The clinical staging and lymph node status of patients with different molecular subtypes of breast cancer were significantly different(P<0.05).There were significant differences in enhancement mode,enhancement amplitude,clear boundary after enhancement,and internal filling defect in patients with different molecular subtypes of breast cancer(P<0.05).The accuracy of CEUS in the diagnosis of molecular subtypes of breast cancer was 87.04%(94/108),which was consistent with the pathological findings(Kappa=0.820).Conclusion CEUS has high accuracy in the diagnosis of molecular subtypes of breast cancer,and its consistency with pathological findings is high,providing a reference for clinical surgery,adjuvant chemotherapy and other treatments.
