摘　　要：目的 探讨肝癌患者血清同型半胱氨酸(Hcy)、胰岛素样生长因子1(IGF1)、红细胞体积分布宽度(RDW)水平及临床意义。方法 选取92例肝癌患者和28例健康体检者,分别作为观察组和对照组,检测并比较两组受试者的血清Hcy、IGF1、RDW水平。比较不同肝功能分级、临床分期肝癌患者的血清Hcy、IGF1、RDW水平。采用Spearman相关分析法分析血清Hcy、IGF1、RDW水平与肝癌患者肝功能分级和临床分期的相关性。结果 观察组患者血清Hcy、RDW水平均明显高于对照组,血清IGF1水平明显低于对照组,差异均有统计学意义(P﹤0.01)。随着肝功能分级和临床分期的升高,肝癌患者血清Hcy、RDW水平均逐渐升高,血清IGF1水平均逐渐降低,差异均有统计学意义(P﹤0.05)。血清Hcy、RDW水平与肝癌患者的肝功能分级和临床分期均呈正相关(P﹤0.01),血清IGF1水平与肝癌患者的肝功能分级和临床分期均呈负相关(P﹤0.01)。结论 肝癌患者血清Hcy、RDW水平升高,血清IGF1水平降低,且这些指标与患者的肝功能分级及临床分期有关,检测这些指标对判断肝癌严重程度及评估预后具有重要的参考意义。Objective To investigate the serum levels of homocysteine(Hcy),insulin-like growth factor 1(IGF1),red cell volume distribution width(RDW)in patients with liver cancer and their clinical significance.Method A total of 92 patients with liver cancer and 28 healthy subjects were selected as observation group and control group,respectively.The serum Hcy,IGF1 and RDW levels were detected and compared between the two groups.The serum Hcy,IGF1 and RDW levels in liver cancer patients with different liver function grades and clinical stages were also compared.Spearman correlation analysis performed to analyze the correlation between serum Hcy,IGF1,RDW levels and liver function grade,clinical stage of patients with liver cancer.Result The serum Hcy and RDW levels in observation group were significantly higher than those in control group,and the serum IGF1 level was significantly lower than that in control group,the differences were statistically significant(P<0.01).With the increase of liver function grade and clinical stage,the serum Hcy and RDW levels of patients with liver cancer were gradually increased,and the serum IGF1 levels were gradually decreased,the differences were statistically significant(P<0.05).The serum Hcy and RDW levels were positively correlated with the liver function grade and clinical stage of patients with liver cancer(P<0.01),and the serum IGF1 level was negatively correlated with the liver function grade and clinical stage of patients with liver cancer(P<0.01).Conclusion The serum Hcy and RDW levels increased and serum IGF1 level decreased in patients with liver cancer,and these indicators were related to the liver function grade and clinical stage of patients.Detection of these indicators has important reference significance for judging the severity of liver cancer and evaluating the prognosis.
