血清C反应蛋白、尿液miRNA-129-5p水平与膀胱癌患者预后的关系

摘　　要：目的探讨血清C反应蛋白(CRP)、尿液miRNA-129-5p水平与膀胱癌患者预后的关系。方法选取97例膀胱癌患者、100例泌尿系良性疾病患者和100例健康体检者,分别作为膀胱癌组、良性组和对照组,比较3组受试者、不同临床特征及不同预后膀胱癌患者的血清CRP、尿液miRNA-129-5p水平。血清CRP水平与尿液miRNA-129-5p水平的相关性采用Pearson相关分析;膀胱癌患者预后的影响因素采用Cox回归分析;绘制受试者工作特征(ROC)曲线,计算曲线下面积(AUC),评估血清CRP、尿液miRNA-129-5p水平单独及联合检测对膀胱癌患者预后的预测价值。结果膀胱癌组患者血清CRP水平高于良性组和对照组,尿液miRNA-129-5p水平低于良性组和对照组,良性组患者血清CRP水平高于对照组,尿液miRNA-129-5p水平低于对照组,差异均有统计学意义(P﹤0.05)。Pearson相关分析显示,膀胱癌患者血清CRP水平与尿液miRNA-129-5p水平呈负相关(r=-0.530,P﹤0.05)。随访3年,97例膀胱癌患者,生存79例,死亡18例,分别作为预后良好组和预后不良组,预后不良组膀胱癌患者血清CRP水平明显高于预后良好组,尿液miRNA-129-5p水平明显低于预后良好组,差异均有统计学意义(P﹤0.01)。多因素Cox分析结果显示,分化程度为低分化、TNM分期为Ⅲ~Ⅳ期、血清CRP水平升高均为膀胱癌患者预后的独立危险因素(P﹤0.05),尿液miRNA-129-5p水平升高为膀胱癌患者预后的独立保护因素(P﹤0.05)。ROC曲线显示,血清CRP、尿液miRNA-129-5p水平联合检测预测膀胱患者预后的AUC为0.981(95%CI:0.930~0.998),高于二者单独检测。结论膀胱癌患者血清CRP水平较高,尿液miRNA-129-5p水平较低,其水平与膀胱癌TNM分期、淋巴结转移、分化程度有关,二者联合检测对膀胱癌患者的预后具有较高的预测价值。Objective To analyze the relationship between serum C-reactive protein,urinary miRNA-129-5p levels and prognosis of bladder cancer patients.Method A total of 97 patients with bladder cancer,100 patients with urinary system benign diseases,and 100 healthy subjects were selected as bladder cancer group,benign group,and control group,respectively.Serum CRP and urinary miRNA-129-5p levels in three groups of subjects,different clinical characteristics and different prognosis of bladder cancer patients were compared.The correlation between serum CRP level and urinary miRNA-129-5p level was analyzed by Pearson correlation analysis.Cox regression analysis was used to analyze the prognostic factors of bladder cancer patients.The receiver operating characteristic(ROC)curve was drawn,the area under the curve(AUC)was calculated,and the predictive value of serum CRP and urinary miRNA-129-5p levels single and combined detection for the prognosis of bladder cancer patients were evaluated.Result The serum CRP level of bladder cancer group was higher than those of benign group and control group,the urinary miRNA-129-5p level was lower than those of benign group and control group,the serum CRP level of benign group was higher than that of control group,and the urinary miRNA-129-5p level was lower than that of control group,and the differences were statistically significant(P<0.05).Pearson correlation analysis showed that serum CRP level was negative correlation with urinary miRNA-129-5p level in bladder cancer patients(r=-0.530,P<0.05).During 3-year follow-up,97 bladder cancer patients survived 79 cases and died 18 cases.They were treated as good prognosis group and poor prognosis group,respectively.The serum CRP level in poor prognosis group was significantly higher than that in good prognosis group,and the urinary miRNA-129-5p level was significantly lower than that in good prognosis group,and the differences were statistically significant(P<0.01).Multivariate Cox analysis showed that poor differentiation,TNM stage III-IV,and el
