摘　　要：目的探讨CT检查联合癌胚抗原(CEA)和鳞状细胞癌抗原(SCCA)对肺癌的诊断效能。方法选取122例肺癌患者作为肺癌组,87例肺部良性病变患者作为良性病变组。比较两组患者CEA、SCCA水平,分析血清CEA、SCCA水平与肺癌的关系。绘制受试者工作特征(ROC)曲线,分析CT、CEA和SCCA单独及联合检查对肺癌的诊断价值。结果122例肺癌组患者中,CT诊断符合率为90.98%(111/122),87例良性病变组患者中,CT诊断符合率为93.10%(81/87),总准确度为91.87%(192/209),Kappa值为0.834(P﹤0.01)。肺癌组患者CEA、SCCA水平均明显高于良性病变组,差异均有统计学意义(P﹤0.01)。ROC曲线显示,CT、CEA、SCCA联合检查诊断肺癌的曲线下面积(AUC)为0.985,高于CT、CEA、SCCA单独检查。结论CT、CEA、SCCA联合检查对肺癌有较高的诊断效能,有良好的应用价值。Objective To explore the diagnostic efficacy of CT combined with carcinoembryonic antigen(CEA)and squamous cell carcinoma antigen(SCCA)in lung cancer(LC).Method A total of 122 patients with LC were selected as LC group,and 87 patients with benign lung disease were selected as benign lung disease group.The levels of CEA and SCCA were compared between the two groups,and the relationship between serum CEA,SCCA levels and LC were analyzed.The receiver operating characteristic(ROC)curve was drawn to analyze the diagnostic value of CT,CEA and SCCA alone and combined examination for LC.Result The diagnostic accuracy of CT in 122 LC patients was 90.98%(111/122),and that in 87 benign lesions was 93.10%(81/87),the total accuracy was 91.87%(192/209),and Kappa value was 0.834(P<0.01).The levels of CEA and SCCA in the LC group were significantly higher than those in the benign lesion group,and the differences were statistically significant(P<0.01).ROC curve showed that the area under the curve(AUC)of CT,CEA and SCCA combined examination in the diagnosis of LC was 0.985,which was higher than that of CT,CEA and SCCA alone examination.Conclusion The combined examination of CT,CEA and SCCA has high efficiency and good application value in the diagnosis of LC.
