摘  要:目的探讨基于失效模式与效应分析(FMEA)模型的质控干预对食管癌根治术患者应激反应及炎症因子水平的影响。方法根据干预方法的不同将126例食管癌根治术患者分为对照组(n=60)和研究组(n=66),对照组患者采用常规干预,研究组患者在对照组的基础上采用基于FMEA模型的质控干预。比较两组患者的术后恢复指标、炎症因子[C反应蛋白(CRP)、白细胞介素-6(IL-6)]水平及术后并发症发生情况。结果研究组患者引流管拔出时间、首次排气时间、首次下床时间、住院时间均短于对照组,术后并发症总发生率低于对照组,差异均有统计学意义(P﹤0.05)。术后1天,两组患者皮质醇、去甲肾上腺素、肾上腺素、CRP、IL-6水平均高于本组术前,研究组患者皮质醇、去甲肾上腺素、肾上腺素、CRP、IL-6水平均低于对照组,差异均有统计学意义(P﹤0.05)。结论基于FMEA模型的质控干预能促进食管癌根治术患者术后恢复,减轻应激反应,减少术后并发症。Objective To explore the effects of quality control intervention based on the failure mode and effect analysis(FMEA)model on stress response and inflammatory factors levels in patients with radical resection of esophageal cancer.Method A total of 126 patients with radical resection of esophageal cancer were divided into control group(n=60)and study group(n=66)according to different intervention methods.Patients in the control group received conventional intervention,while patients in the study group received quality control intervention based on FMEA model on the basis of the control group.Postoperative recovery indicators,inflammatory factors[C-reactive protein(CRP),interleukin-6(IL-6)]levels,and the incidence of postoperative complications were compared between the two groups.Result The drainage tube extraction time,first exhaust time,first time to get out of bed and length of hospital stay in the study group were shorter than those in the control group,and the total incidence of postoperative complications was lower than that in the control group,the differences were statistically significant(P<0.05).One day after surgery,the levels of cortisol,norepinephrine,epinephrine,CRP and IL-6 in both groups were higher than those before surgery,while the levels of cortisol,norepinephrine,epinephrine,CRP and IL-6 in the study group were lower than those in the control group,the differences were statistically significant(P<0.05).Conclusion Quality control intervention based on FMEA model can promote the postoperative recovery of patients with radical resection of esophageal cancer,reduce stress response and postoperative complications.
