摘　　要：目的探讨腹横肌平面阻滞(TAPB)联合纳布啡在老年结直肠癌腹腔镜手术患者中的应用效果。方法选取69例老年结直肠癌腹腔镜手术患者,根据术后镇痛方式的不同分为对照组35例和观察组34例,对照组患者术后采用TAPB联合罗哌卡因的镇痛方案,观察组患者术后采用TAPB联合纳布啡+罗哌卡因的镇痛方案。比较两组患者的术后疼痛[视觉模拟评分法(VAS)]、术后恢复及并发症发生情况。结果术后6、9、12 h,两组患者VAS评分均高于本组术后3 h,观察组患者VAS评分均低于对照组,差异有统计学意义(P﹤0.05)。观察组患者肛门排气时间、下床活动时间及导管拔除时间均短于对照组,并发症总发生率低于对照组,差异均有统计学意义(P﹤0.05)。结论TAPB联合纳布啡镇痛应用于老年结直肠癌腹腔镜手术患者中效果显著,可减轻术后疼痛,促进患者术后恢复,且安全性较高。Objective To explore the application effect of transversus abdominis plane block(TAPB)combined with nalbuphine in elder patients undergoing laparoscopic surgery for colorectal cancer.Method Sixty-nine elderly patients who underwent laparoscopic surgery for colorectal cancer were selected and divided into control group of 35 cases and observation group of 34 cases according to different postoperative analgesic methods.The control group received postoperative analgesia with TAPB combined with ropivacaine,while the observation group received postoperative analgesia with TAPB combined with nalbuphine+ropivacaine.Postoperative pain[visual analogue scale(VAS)],postoperative recovery,and complications were compared between the two groups.Result At 6,9 and 12 h after surgery,the VAS scores of both groups were higher than those at 3 h after surgery,and the VAS scores of observation group were lower than those of control group,and the differences were statistically significant(P<0.05).The time of anal exhaust,the time of getting out of bed and the time of catheter removal in the observation group were shorter than those in the control group,and the total incidence of complications was lower than that in the control group,and the differences were statistically significant(P<0.05).Conclusion TAPB combined with nalbuphine analgesia has a significant effect in elderly patients undergoing laparoscopic surgery for colorectal cancer,which can reduce postoperative pain and promote postoperative recovery of patients with high safety.
