大分割放疗联合信迪利单抗治疗结直肠癌患者的临床疗效

摘　　要：目的探讨大分割放疗联合信迪利单抗治疗结直肠癌患者的临床疗效。方法根据治疗方法的不同将80例结直肠癌患者分为对照组(n=42,常规分割放疗联合信迪利单抗治疗)和观察组(n=38,大分割放疗联合信迪利单抗治疗)。比较两组患者的临床疗效、免疫功能指标(CD3^(+)、CD4^(+)、CD8^(+)、CD4^(+)/CD8^(+))及不良反应发生情况。结果观察组患者的客观缓解率和疾病控制率均高于对照组,差异均有统计学意义(P﹤0.05)。治疗后,两组患者CD3^(+)、CD4^(+)水平及CD4^(+)/CD8^(+)均升高,CD8^(+)水平均降低,观察组患者CD3^(+)、CD4^(+)水平及CD4^(+)/CD8^(+)均高于的对照组,CD8^(+)水平低于对照组,差异均有统计学意义(P﹤0.05)。两组患者恶心呕吐、腹泻、血小板减少及甲状腺功能减退发生率比较,差异均无统计学意义(P﹥0.05)。结论大分割放疗联合信迪利单抗可有效提高结直肠癌患者的疾病控制率,改善免疫功能,且不增加不良反应发生率。Objective To explore the clinical efficacy of hypofractionated radiotherapy combined with sintilimab in the treatment of colorectal cancer patients.Method According to different treatment methods,80 patients with colorectal cancer were divided into control group(n=42,conventional fractionated radiotherapy combined with sintilimab)and observation group(n=38,hypofractionated radiotherapy combined with sintilimab).The clinical efficacy,immune function indexes(CD3^(+),CD4^(+),CD8^(+),CD4^(+)/CD8^(+))and adverse reactions were compared between the two groups.Result The objective response rate and disease control rate of observation group were higher than those of control group,and the differences were statistically significant(P<0.05).After treatment,the levels of CD3^(+),CD4^(+)and CD4^(+)/CD8^(+)were increased in both groups,while the levels of CD8^(+)were decreased in both groups,the levels of CD3^(+),CD4^(+)and CD4^(+)/CD8^(+)in the observation group were higher than those in the control group,and the level of CD8^(+)was lower than that in the control group,the differences were statistically significant(P<0.05).There were no significant differences in the incidences of nausea,vomiting,diarrhea,thrombocytopenia and hypothyroidism between the two groups(P>0.05).Conclusion Hypofractionated radiotherapy combined with sintilimab can effectively improve the disease control rate and immune function in patients with colorectal cancer,and do not increase the incidence of adverse reactions.
