摘　　要：目的探讨不同入路方式腹腔镜前列腺癌根治术对患者排尿功能的影响。方法根据手术方式的不同将59例前列腺癌患者分为对照组(n=29)和观察组(n=30),对照组患者接受经腹腔入路LRP术治疗,观察组患者接受经腹膜外入路LRP术治疗。比较两组患者的手术相关指标、排尿功能、并发症发生情况及控尿情况。结果观察组患者术中出血量少于对照,导尿管留置时间、住院时间及手术时间均短于对照组,差异均有统计学意义(P﹤0.05)。术后,两组患者最大尿流率均高于本组术前,观察组患者最大尿流率高于对照组,差异均有统计学意义(P﹤0.05)。观察组患者的并发症总发生率为10.00%,与对照组患者的6.90%比较,差异无统计学意义(P﹥0.05)。术后1、3、6个月,观察组患者的控尿率均高于对照组,差异均有统计学意义(P﹤0.05)。结论与经腹腔入路相比,前列腺癌患者接受经腹膜外入路LRP术可促进术后恢复,减轻手术创伤,提高患者的排尿功能及控尿率,有临床应用价值。Objective To investigate the influence of laparoscopic radical prostatectomy(LRP)with different approaches on urinary function in patients with prostate cancer.Method According to different surgical methods,59 prostate cancer patients were divided into control group(n=29)and observation group(n=30).Patients in the control group received intraperitoneal approach LRP treatment,while patients in the observation group received extraperitoneal approach LRP treatment.The surgical related indicators,urinary function,incidence of complications,and urinary control were compared between the two groups.Result The intraoperative bleeding volume in observation group was less than that in control group,and the catheterization retention time,hospitalization time,and surgery time were shorter than those in control group,and the differences were statistically significant(P<0.05).After surgery,the maximum urinary flow rate in both groups were higher than those before surgery,the maximum urinary flow rate in observation group was higher than that in control group,and the differences were statistically significant(P<0.05).The total incidence of complications in the observation group was 10.00%,which was not significantly different from 6.90% in the control group(P>0.05).At 1,3,and 6 months after surgery,the urinary control rate in observation group were higher than those in control group,and the differences were statistically significant(P<0.05).Conclusion Compared with the intraperitoneal approach,LRP surgery through the extraperitoneal approach can promote postoperative recovery,reduce surgical trauma,improve urinary function and urinary control rate in prostate cancer patients,and has clinical application value.
