丙戊酸钠联合左乙拉西坦治疗老年脑卒中后癫痫的临床研究

摘　　要：目的研究丙戊酸钠、左乙拉西坦联合用于临床治疗老年脑卒中后癫痫疾病的价值。方法选取2022年5月—2023年12月宜兴市人民医院收治老年脑卒中后癫痫患者126例为研究对象,以不同治疗方法分组,对照组(63例)单独用药丙戊酸钠治疗;观察组(63例)联合用药丙戊酸钠、左乙拉西坦治疗。对比治疗效果,癫痫发作频率及发作持续时间,检测脑电图功率谱及炎症因子,比较两组患者用药后不良反应。结果观察组治疗总有效率为96.83%(61/63),高于对照组84.13%(53/63),差异有统计学意义(χ^(2)=5.895,P<0.05)。观察组癫痫发作频率较低,持续时间较短,差异均有统计学意义(P均<0.05)。脑电图功率谱观察组治疗后结果较高,炎症因子观察组治疗后较低,差异均有统计学意义(P均<0.05)。两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论临床治疗老年脑卒中后癫痫疾病可在丙戊酸钠基础上联用左乙拉西坦,效果更明显,有效减少癫痫发作,改善脑电波,且不增加不良反应。Objective To study the value of sodium valproate combined with levetiracetam in the clinical treatment of epilepsy after stroke in the elderly.Methods A total of 126 elderly patients with epilepsy after stroke admitted to Yixing People's Hospital from May 2022 to December 2023 were selected as the research objects.They were divided into two groups according to different treatment methods.The control group(63 cases)was treated with sodium valproate alone,the observation group(63 cases)was treated with sodium valproate and levetiracetam.The therapeutic effect,epileptic seizure frequency and duration was compared,the electroencephalogram power spectrum and inflammatory factors were detected,and the adverse reactions of the two groups were compared.Results The total effective rate of the observation group was 96.83%(61/63),which was higher than that of the control group 84.13%(53/63),and the difference was statistically significant(χ^(2)=5.895,P<0.05).The frequency of epileptic seizures in the observation group was lower and the duration was shorter,the differences were statistically significant(both P<0.05).The results of electroencephalogram power spectrum in the observation group were higher after treatment,and the inflammatory factors in the observation group were lower after treatment,the differences were statistically significant(both P<0.05).There was no significant difference in the total incidence of adverse reactions between the two groups(P>0.05).Conclusion The clinical treatment of epilepsy after stroke in the elderly can be combined with levetiracetam on the basis of sodium valproate.The effect is more obvious,effectively reducing seizures,improving brain waves,and not increasing adverse reactions.
