高分辨率磁共振血管壁成像在脑动脉夹层分离诊断中的价值分析

摘　　要：目的分析高分辨率磁共振血管壁成像在脑动脉夹层分离诊断中的价值。方法选取2022年1—12月青岛市黄岛区中医医院收治的43例疑似脑动脉夹层分离患者为研究对象。所有患者均采用高分辨率磁共振血管壁成像进行检查,以数字减影血管造影检查结果为诊断金标准,分析高分辨率磁共振血管壁成像对脑动脉夹层分离的诊断效能及诊断符合率。结果43例疑似脑动脉夹层分离患者中,阳性21例,阴性22例。高分辨率磁共振血管壁成像诊断的灵敏度为95.24%(20/21)、特异度为100.00%(22/22)、准确度为97.67%(42/43),kappa值为0.953。高分辨率磁共振血管壁成像对椎动脉夹层的诊断符合率为90.00%(9/10),对颈动脉夹层的诊断符合率为100.00%(11/11)。结论高分辨率磁共振血管壁成像诊断脑动脉夹层分离的诊断效能及诊断符合率较高,且与数字减影血管造影检查的一致性较高。Objective To analyze the value of high-resolution magnetic resonance vascular wall imaging in the diagnosis of cerebral artery dissection separation.Methods From January to December 2022,a total of 43 patients with suspected cerebral artery dissection separation admitted to Huangdao District Hospital of Tradi-tional Chinese Medicine in Qingdao were selected as the research objects.All patients were examined by high-resolution magnetic resonance vascular wall imaging.The results of digital subtraction angiography were used as the gold standard for diagnosis.The diagnostic efficacy and diagnostic coincidence rate of high-resolution mag-netic resonance vascular wall imaging for cerebral artery dissection separation were analyzed.Results Among the 43 patients with suspected cerebral artery dissection separation,21 were positive and 22 were negative.The sensitivity,specificity,accuracy of high-resolution magnetic resonance vascular wall imaging were 95.24%(20/21),100.00%(22/22),97.67%(42/43),respectively,kappa value was 0.953.The coincidence rate of high-resolution magnetic resonance vascular wall imaging in the diagnosis of vertebral artery dissection was 90.00%%(9/10),and the coincidence rate of carotid artery dissection was 100.00%(11/11).Conclusion The di-agnostic efficiency and diagnostic coincidence rate of high-resolution magnetic resonance vascular wall imag-ing in the diagnosis of cerebral artery dissection separation are high,and the consistency with digital subtraction angiography is high.
