摘 要:目的了解兰州市鼠伤寒沙门菌的分子分型特征和耐药特点。方法对2019—2022年收集的113株食源性菌株开展生化和血清学鉴定,对分离到的鼠伤寒沙门菌进行脉冲场凝胶电泳(PFGE)分子分型并对14种药物开展敏感试验。结果兰州市食源性腹泻的主要致病菌为沙门菌、占88.49%,其中鼠伤寒沙门菌是主要血清型之一;鼠伤寒沙门菌PFGE分子分型结果显示,主要分成A和B两个聚类簇,相似度分别为61.82%和47.20%,共得到23个带型,其中仅有两个带型各含2个菌株(100%同源),均分布在2021年;药物敏感试验结果显示,四环素的耐药率最高(92.0%),对头孢西丁和亚胺培南两种药物100.00%敏感;对3种及以上抗生素耐药的构成比高达83.3%,AMP-TET-CHL-SXT耐药谱的构成比最高(16.67%)。结论兰州市食源性鼠伤寒沙门菌PFGE图谱呈多态性分布,存在年度聚集性,多重耐药情况严峻;应加强抗生素耐药危害的宣传,对临床治疗使用和畜牧业农业生产的抗生素加强监督监管。Objective To understand the characteristics of molecular typing and drug resistance of salmonella typhimurium in Lanzhou city.Methods A total of 113 food-borne strains were collected and identified by biochemical and serological methods from 2019 to 2022.The salmonella typhimurium was isolated and molecular classified by pulsed field gel electrophoresis(PFGE),and the susceptibility test was carried out for 14 kinds of drugs.Results salmonella was the main pathogenic bacteria of food-borne diarrhea in Lanzhou city,accounting for 88.49%,and salmonella typhimurium was one of the main serotypes.The results of PFGE molecular classification of salmonella typhimurium showed two clusters of A and B were the main clusters,and the similarities of them was 61.82%and 47.20%,respectively.A total of 23 banding patterns were obtained,and there were only 2 bands with 2 strains each(100%homologous)in 2021.The results of drug sensitivity test showed the resistance rate of tetracycline was the highest(92.0%),and the sensitivity was 100%to cefoxitin and imipenem.The proportion of resistance to three and more antibiotics reached up to 83.3%.The proportion of resistant spectrum of AMPTET-CHL-SXT was the highest(16.67%).Conclusions The PFGE map of food-borne salmonella typhimurium showed polymorphism distribution and annual aggregation.The multi-drug resistance is severe in Lanzhou.The publicity on the hazard of antibiotic resistance should be strengthened,and the supervision of antibiotics used in clinical treatment,animal husbandry and agricultural production should be strengthened.
