摘 要:目的探讨肉毒毒素注射联合减重平板步行训练(BWSTT)对痉挛型脑瘫(SCP)患儿下肢运动功能、平衡能力和步态的影响。方法随机将2022年5月至2024年2月在郑州大学第三附属医院就诊的70例SCP患儿均分为两组,各35例。对照组进行BWSTT,在此基础上给予观察组肉毒毒素注射治疗。比较两组治疗前后Fugl-Meyer运动功能评估量表-下肢(FMA-LE)、Berg平衡量表(BBS)评分和步态(步长、步速和步宽)。结果治疗后,两组FMA-LE评分均上升,且观察组治疗后FMA-LE评分较高(P<0.05);治疗后,两组BBS评分均上升,且观察组治疗后BBS评分较高(P<0.05);治疗后,两组步长、步速水平均上升,而步宽水平下降,且治疗后观察组步长、步速水平较高,而步宽水平较低(P<0.05)。结论肉毒毒素注射联合BWSTT可有效改善SCP患儿下肢运动功能、平衡能力和步态,该治疗方案具有较大的临床应用前景。Objective To investigate the effect of botulinum toxin injection combined with body-weight supported treadmill training(BWSTT)on lower limb motor function,balance ability and gait in children with spastic cerebral palsy(SCP).Methods A total of 70 children with SCP admitted to the Third Affiliated Hospital of Zhengzhou University from May 2022 to February 2024 were randomly divided into two groups with 35 patients each.The control group was treated with BWSTT,and the observation group was treated with botulinum toxin injection on this basis.The scores of Fugl-Meyer assessment-lower extremity(FMA-LE),Berg balance scale(BBS)and gait(step length,step speed and step width)were compared between the two groups before and after treatment.Results After treatment,the FMA-LE scores were increased in the two groups,and the FMA-LE score in the observation group was higher(P<0.05).After treatment,the BBS scores were increased in the two groups,and the BBS score in the observation group was higher(P<0.05).After treatment,the levels of step length and speed increased,while the level of step width decreased in the two groups,and the levels of step length and speed were higher,while the level of step width was lower in the observation group(P<0.05).Conclusion Botulinum toxin injection combined with BWSTT can effectively improve the lower limb motor function,balance ability,and gait in children with SCP,and this treatment has a great clinical application prospect.
