摘  要:目的探讨血清缺氧诱导因子-1α(hypoxia-inducible factor-1α,HIF-1α)与脑微出血(cerebral microbleeds,CMBs)及认知功能障碍的相关性,评估HIF-1α对脑小血管病(cerebral small vessel disease,CSVD)相关认知功能障碍的预测价值。方法纳入自2022年6月至2023年11月就诊于新乡医学院第一附属医院神经内科的CSVD患者104例,对入组患者进行基本资料统计、头颅核磁检查、认知功能评估、血清HIF-1α检测,统计入组患者CMBs的数量和部位,并进行分组,研究HIF-1α与认知功能和CMBs的相关性,分析CMBs、认知功能障碍的影响因素,评估HIF-1α对认知功能障碍发生的预测价值。结果不同CMBs严重程度在HIF-1α、认知功能的差异有统计学意义,血清HIF-1α浓度与视空间与执行功能、注意力、延迟回忆呈负相关,与CMBs数量呈正相关,HIF-1α可能是CMBs和CSVD相关认知功能障碍的危险因素,且对CSVD相关认知功能障碍具有潜在的预测价值。结论血清HIF-1α浓度与CMBs数量及CSVD相关认知功能障碍有关,血清HIF-1α浓度可能对CSVD相关认知功能障碍有一定的预测价值。Objective To explore the correlation between serum hypoxia-inducible factor-1α(HIF-1α)levels and cerebral microbleeds(CMBs)and cognitive impairment and to assess the predictive value of HIF-1αfor CSVD-related cognitive impairment.Methods A total of 104 patients with CSVD who attended the Department of Neurology,First Affiliated Hospital of Xinxiang Medical University from June 2022 to November 2023 were enrolled.All enrolled patients were subjected to basic statistics,cranial nuclear magnetic resonance examination,cognitive function assessment,and serum HIF-1αtest,and the number and location of CMBs were counted.Based on the above data the enrolled patients were grouped.The correlation between HIF-1αand cognitive function and CMBs was studied the influencing factors of CMBs and cognitive impairment were analyzed,and the predictive value of HIF-1αon the occurrence of cognitive impairment was evaluated.Results There were statistically significant differences in HIF-1αlevels and cognitive function among different CMBs groups.Serum HIF-1αlevels were significantly negatively correlated with overall cognitive function,visuospatial and executive function,attention,and delayed recall,and serum HIF-1αwas positively correlated with the number of CMBs.HIF-1αmay be a risk factor for CMBs and cognitive impairment associated with CSVD,and serum HIF-1αhas potential in predict the cognitive impairment caused by CSVD.Conclusion Serum levels of HIF-1αwere associated with the number of CMB and CSVD-related cognitive impairment,and serum levels of HIF-1αmay have a predictive value for CSVD-related cognitive impairment.
