3h视频脑电图在小儿癫痫及其他发作性疾病的诊断价值

摘　　要：目的探讨3 h视频脑电图(V-EEG)在小儿癫痫及其他发作性疾病的诊断价值。方法选择2021年1月至2023年12月在蚌埠市第一人民医院接受治疗的有发作性症状患儿80例,均进行3 hV-EEG和常规脑电图(EEG)检查。比较两组检查结果、相关波率、3 hV-EEG放电时特征、发作情况。结果3 hV-EEG监测异常、痫样放电及同步发作检出率均显著高于常规EEG(P<0.05);监测发现,睡眠期、清醒期及睡眠期和清醒期均出现痫样放电,睡眠期痫样放电检出率显著高于清醒期,比较差异有统计学意义(P<0.05);在监测过程中,总共发现26例发作,其中伴有发作期痫样放电10例(38.46%),临床确诊为癫痫性发作,无同步异常放电的临床发作16例(61.54%),判断为非癫痫性发作;80例患者监测结果结合病史可以明确诊断癫痫62例,余18例为可疑癫痫。结论3 hV-EEG在小儿癫痫及其他发作性疾病中监测效果较好,为临床诊断癫痫提供重要依据。Objective To study Diagnostic value of 3h video electroencephalogram(V-EEG)in children with epilepsy and other epileptic diseases.Methods 80 children with episodic symptoms who were treated in Bengbu First People's Hospitalfrom January 2021 to December 2023 were selected for 3h V-EEG and routine EEG examination.The examination results,correlation wave rate,3h V-EEG discharge characteristics and seizure were compared between the two groups.Results The detection rates of abnormal,epileptic discharge and synchronous seizure in 3h V-EEG were significantly higher than those in conventional EEG(P<0.05).It was found that epileptic discharge occurred in sleep period,wake period,sleep period and wake period,and the detection result of epileptic discharge in sleep period was significantly higher than that in wake period,and the comparative difference was significant(P<0.05).In the monitoring process,a total of 26 seizures were found,of which 10 cases(38.46%)were accompanied by epileptic discharge,clinically diagnosed as epileptic seizures,and 16 cases(61.54%)were clinically diagnosed as non-epileptic seizures without abnormal synchronous discharge.Of the 80 patients,62were diagnosed with epilepsy and 18 were suspected of epilepsy.Conclusion 3 h V-EEG is effective in monitoring epilepsy and other epileptic diseases in children,and provides an important basis for clinical diagnosis of epilepsy.
