摘  要:目的 基于RBRVS(基于资源的相对价值评估)理论及医院实际构建护理绩效分配方案,并评价绩效改革实践效果。方法 2021-2022年,医院以岗位基础绩效工资为保障,以RBRVS为评价手段,以成本管控为基础,构建了包含岗位基础绩效、核算绩效、专项奖励绩效、夜班绩效、科研创新绩效和年终绩效的“六位一体”绩效分配方案;并以工作量考核为基础、质量考核为重点,结合护理、教学、科研、管理、专科建设5个维度的综合目标进行绩效考核管理。分别从护士及患者角度评价绩效改革实施效果。结果 绩效改革实施后,护士的工作满意度有所提升,离职率明显降低,住院患者的满意度也明显提升,患者平均住院日有所缩短,护理科研及管理指标呈向好发展,管理成效明显。结论 “六位一体”的护理绩效分配方案可有效调动护士工作积极性、提高护理工作效率、提升护理服务质量,有利于推动护理管理的精细化及规范化发展。Objective To develop a nursing performance distribution scheme based on RBRVS theory and hospital-specific conditions,and to evaluate the effect of performance reform.Methods From 2021 to 2022,the hospital developed a"six-in-one"performance distribution scheme,integrating basic position performance,accounting performance,special reward performance,night shift performance,scientific research and innovation performance and year-end performance.The scheme was designed to guarantee a basic wage while considering cost control,utilizing RBRVS as an evaluation tool.Performance assessment was grounded in workload assessment,with a focus on quality assessment,incorporating comprehensive objectives across 5 dimensions:nursing,teaching,scientific research,management and specialty development.The effect of performance reform was evaluated from both nurse and patient perspectives.Results After the performance reform,nurses'job satisfaction increased,the turnover rate significantly decreased,inpatient satisfaction significantly increased,the average length of stay shortened,and nursing research and management indicators showed positive trends,demonstrating a substantial impact on management effectiveness.Conclusion The"six-in-one"nursing performance distribution scheme enhances nurses'work motivation,nursing efficiency and nursing quality.It effectively contributes to the refinement and standardization of nursing management.
