摘 要:目的观察补肾活血固齿方对牙周炎大鼠牙周组织和外周血中炎性细胞因子表达的影响,明确其治疗牙周炎的机制。方法将30只雄性SD大鼠随机分为空白组、牙周炎组、补肾活血固齿方组,每组10只。牙周炎组和补肾活血固齿方组大鼠采用牙周结扎联合喂食高糖水和黏软食物的方法建立牙周炎模型。验证造模成功后每组各剩余8只大鼠,补肾活血固齿方组给予补肾活血固齿方3.8 g/kg灌胃,空白组和牙周炎组给予等量生理盐水灌胃,均2次/d。连续灌胃4周后,观察大鼠牙龈情况,RT-PCR法检测牙周组织中白细胞介素-1β(IL-1β)、肿瘤坏死因子-α(TNF-α)、基质金属蛋白酶-8(MMP-8)mRNA表达情况,酶联免疫吸附测定法检测外周血中IL-1β、TNF-α、超敏C反应蛋白(hs-CRP)水平。结果空白组、补肾活血固齿方组大鼠牙龈无充血,触之无出血;牙周炎组大鼠牙龈红肿、质地松软,触之出血。牙周炎组大鼠牙周组织中IL-1β、TNF-α、MMP-8 mRNA相对表达量和外周血中IL-1β、TNF-α、hs-CRP水平均明显高于空白组(P均<0.05);补肾活血固齿方组大鼠牙周组织中IL-1β、TNF-α、MMP-8 mRNA相对表达量和外周血中IL-1β、TNF-α、hs-CRP水平均明显低于牙周炎组(P均<0.05),与空白组比较差异均无统计学意义(P均>0.05)。结论补肾活血固齿方可有效治疗牙周炎,机制可能与下调牙周组织中IL-1β、TNF-α、MMP-8 mRNA表达和降低外周血中IL-1β、TNF-α、hs-CRP水平而减轻牙周组织和机体的炎性反应有关。Objective It is to observe the effects of decoction for tonifying kidney,activating blood and strengthening teeth(DTKABST)on expressions of inflammatory cytokines in periodontal tissues and peripheral blood of rats with periodontitis,and to clarify its mechanism in the treatment of periodontitis.Methods Thirty male SD rats were randomly divided into blank group,periodontitis group and DTKABST group,with 10 rats in each group.The rats in the periodontitis group and DTKABST group were modeled by periodontal ligation combined with feeding high sugar water and soft-sticky food to establish models of periodontitis.After verifying the success of modeling,8 rats were left in each group,the DTKABST group was given DTKABST 3.8 g/kg by gavage,while the blank group and periodontitis group were given an equal amount of saline by gavage,all twice daily.After 4 weeks of continuous gavage,the gingival condition of the rats was observed,the mRNA expressions of interleukin-1β(IL-1β),tumor necrosis factor-α(TNF-α),and matrix metalloproteinase-8(MMP-8)in periodontal tissues were detected by RT-PCR,and the levels of IL-1β,TNF-αand high-sensitive C-reactive protein(hs-CRP)in peripheral blood were detected by enzyme-linked immunosorbent assay.Results The gingiva of rats in the blank group and DTKABST group were not congestive without bleeding when touched;the gingiva of rats in the periodontitis group was red and swollen,soft and with bleeding when touched.The mRNA relative expressions of IL-1β,TNF-α,MMP-8 in periodontal tissues and the levels of IL-1β,TNF-αand hs-CRP in peripheral blood of rats in the periodontitis group were significantly higher than those in the blank group(all P<0.05);the mRNA relative expressions of IL-1β,TNF-α,MMP-8 in periodontal tissues and the levels of IL-1β,TNF-αand hs-CRP in peripheral blood of rats in the DTKABST group were significantly lower than those in the periodontitis group(all P<0.05),and the differences were not statistically significant between the DTKABST group and blank group(all
