广泛性焦虑症静息态脑功能局部一致性及低频振幅初步研究

摘　　要：目的运用静息态功能磁共振成像(rs-fMRI)技术,观察广泛性焦虑症(GAD)局部一致性(ReHo)和低频振幅(ALFF)的异常改变特点,同时探究其与临床症状的相关性。方法选取27例GAD患者作为研究组,并将性别、年龄、受教育程度与之匹配的健康人群作为对照组。收集两组的临床资料与rs-fMRI数据,对比两组ReHo和ALFF的异常改变情况,进一步分析差异脑区与临床症状的相关性。结果与对照组比较,研究组右侧额中回、右侧颞中回、右侧枕上回的ReHo增高,右侧额中回、右侧颞中回、左侧枕上回、右侧角回的ALFF增高。相关性分析结果显示,研究组右侧颞中回的ALFF值与HAMA-14评分呈负相关(P<0.001)。结论GAD患者在认知控制网络、默认网络、视觉网络的脑功能活动有异常改变,本研究对理解GAD的神经病理机制有一定参考价值。Objective To observe the characteristics of abnormal changes in regional homogeneity(ReHo)and amplitude of low-frequency fluctuation(ALFF)in generalized anxiety disorder(GAD)using resting-state functional magnetic resonance imaging(rs-fMRI)technology,and simultaneously explore their correlations with clinical symptoms.Methods A total of 27 patients with GAD were selected as study group,and healthy individuals whose gender,age,and education level were matched with study group were taken as control group.The clinical data and rs-fMRI data of both groups were collected to compare the abnormal changes in ReHo and ALFF between two groups,and to further analyze the correlations between the differential brain regions and clinical symptoms.Results Compared with control group,the ReHo values in right middle frontal gyrus,right middle temporal gyrus,and right superior occipital gyrus in study group were increased,and the ALFF values in right middle frontal gyrus,right middle temporal gyrus,left superior occipital gyrus,and right angular gyrus in study group were also increased.Correlation analysis showed that the ALFF value in right middle temporal gyrus of study group was negatively correlated with the HAMA-14 score(P<0.001).Conclusion There are abnormal changes in the brain functional activities of patients with GAD in the cognitive control network,default network,and visual network.This study has certain reference value for understanding the neuropathological mechanism of GAD.
