摘 要:目的探讨语言发育迟缓儿童神经心理发展轨迹与家长社交语言养育行为的动态关系。方法回顾性分析120例语言发育迟缓儿童,利用潜类别增长模型(LCGM)探究语言发育迟缓儿童神经心理发展的动态轨迹;构建多因素Logistic模型,分析影响患儿神经心理发展轨迹的关键因素;运用线性回归分析方法探究患儿神经心理发展水平与父母社交语言养育行为之间的关联性。结果采用LCGM对数据进行分析,将语言发育迟缓儿童神经心理发展轨迹分为高水平缓慢下降组与低水平快速下降组。两组患儿在性别、是否独生子女、留守经历、家族史、主要带养人、每日电子产品使用时间、父母文化程度、每日陪伴看书/讲故事时间、使用有声读物教儿童语言、父母婚姻状况、社交养育行为评分、语言养育行为评分以及总评分比较,差异均具有统计学意义(P<0.05)。多因素Logistic回归分析结果表明,不同主要带养人、每日电子产品使用时间、父母文化程度以及每日陪伴看书/讲故事时间是患儿神经心理高水平缓慢降低发展轨迹的独立危险因素(P<0.05);多元线性回归分析结果表明,随着儿童神经心理发展为高水平-缓慢下降趋势,家长社交语言养育行为评分升高的风险也将增加(P<0.05)。结论语言发育迟缓儿童神经心理发展轨迹与家长社交语言养育行为存在正向关系,不仅为临床干预提供了重要的理论依据,还有助于促进儿童的全面发展、预防长期问题以及提升家庭幸福感。Objective To explore the dynamic relationship between the neuropsychological development trajectory and parental social language parenting behavior in children with speech delay.Methods A total of 120 children with speech delay were selected and retrospectively analyzed with latent class growth modeling(LCGM)to explore their dynamic trajectories of neuropsychological development.A multivariate logistic model was constructed to explore the key factors influencing their neuropsychological development trajectories,and linear regression analysis was conducted to explore the correlation between the neuropsychological development level of children with speech delay and their parents'social language parenting behaviors.Results By analyzing the data using LCGM,the neuropsychological development trajectories of children with speech delay were classified into a high-level slow decline group and a low-level rapid decline group.There existed significant differences in genders,whether only child or not,left-behind experiences,family histories,primary caregivers,daily electronic products use time,parental educational levels,daily time spent reading/telling stories,whether using audio books or not,parental marital status,social-linguistic parenting behavior scores between high-level slow decline group and low-level rapid decline group(P<0.05).The results of multivariate logistic regression analysis showed that different primary caregivers,daily electronic products use time,parental educational levels,and daily time spent reading/telling stories were independent risk factors for the high-level slow decline neuropsychological development trajectory in these children(P<0.05).The results of multiple linear regression analysis indicated a positive correlation between the children's neuropsychological development level and their parents'social-linguistic parenting behavior scores(P<0.05).Conclusion There is a positive correlation between the neuropsychological development trajectory and parental social language parenting behavior in
