摘 要:目的探讨儿童生理性膝内翻的影响因素。方法选取2019年11月—2023年11月在我院儿童保健科确诊的68例生理性膝内翻儿童,设为生理性膝内翻组;选择同期门诊就诊的108例健康儿童,设为对照组。记录并比较两组儿童年龄、身高、体质量的Z评分,以及大运动发育月龄、双膝关节间距、血液学指标等;采用logistic回归法分析上述有差异的指标,得出生理性膝内翻发生的危险因素。结果两组儿童的年龄体质量Z评分、年龄身高Z评分、年龄BMI Z评分、独坐月龄、靠家具站立月龄、辅助行走月龄、独站月龄、独走月龄、双膝关节间距均差异显著(t=-29.427~19.674,P<0.001)。Logistic回归分析显示,年龄BMI Z评分高及独走月龄小是儿童生理性膝内翻的独立危险因素(P<0.05)。结论年龄BMI Z评分高及独走月龄小均为儿童生理性膝内翻的危险因素,及早发现并处理能够降低儿童生理性膝内翻的发生率。Objective To investigate the influencing factors for physiological genu varum in children.Methods A total of 68 children with physiological genu varum who were diagnosed in Department of Child Healthcare in our hospital from November 2019 to November 2023 were enrolled as physiological genu varum group,and 108 healthy children who attended the outpatient service during the same period of time were enrolled as control group.The two groups were compared in terms of Z-scores of age,height and weight,age of gross motor development,the distance between two knee joints,and hematological parameters,and a logistic regression analysis was performed for indicators with statistical significance to identify the risk factors for physiological genu varum.Results There were significant differences between the two groups in weight-for-age Z-score,height-for-age Z-score,BMI-for-age Z-score,age of sitting without support,age of standing against furniture,age of walking with support,age of standing without support,age of walking without support,and the distance between two knee joints(t=-29.427-19.674,P<0.001).The logistic regression analysis showed that a high BMI-for-age Z-score and a young age of walking without support were independent risk factors for physiological genu varum in children(P<0.05).Conclusion A high BMI-for-age Z-score and a young age of walking without support are risk factors for physiological genu varum in children,and early identification and intervention can help reduce the incidence rate of physiological genu varum in children.
