摘 要:目的:探究曲克芦丁脑蛋白水解物联合尼麦角林对急性缺血性脑卒中(AIS)患者的影响。方法:选取2023年1月至2023年12月本院诊疗AIS患者160例,根据治疗方法不同分为对照组、观察组。其中予以尼麦角林+曲克芦丁脑蛋白水解物的80例为观察组,予以尼麦角林的80例为对照组。比较两组临床疗效、不良反应发生率、治疗前后脑血液流变学[大脑中动脉(MCA)的舒张末期血流速度(Vd)、收缩期峰值血流速度(Vp)、平均血流速度(Vm)]、氧化应激[超氧化物歧化酶(SOD)、晚期氧化蛋白产物(AOPP)、丙二醛(MDA)]及相关血清因子[神经生长因子(NGF)、神经元特异性烯醇化酶(NSE)、神经营养因子(NTF)]水平。结果:治疗后观察组脑血液流变学水平高于对照组(P<0.05);两组临床总有效率比较,差异显著(87.50%比75.00%,P<0.05);观察组治疗后SOD水平升高幅度大于对照组,AOPP、MDA水平降低幅度小于对照组(P<0.05);治疗后观察组NTF水平高于对照组,NSE、NGF水平低于对照组(P<0.05);两组不良反应发生率比较,无显著差异(16.25%比8.75%,P>0.05)。结论:曲克芦丁脑蛋白水解物联合尼麦角林治疗AIS疗效明显,可有效改善脑血液流变学,调节体内氧化应激反应,修复神经元,安全性较高。Objective:To investigate the effects of troxrutin brain protein hydrolysate combined with niergoline on patients with acute ischemic stroke(AIS).Method:160 patients with AIS were selected from January 2023 to December 2023,and divided into control group and observation group according to different treatment methods.Among them,80 cases receiving niergoline+troxrutin brain protein hydrolysate were observation group and 80 cases receiving niergoline were control group.The clinical efficacy,incidence of adverse reactions,cerebral hemorheology(end-diastolic blood flow velocity(Vd),peak systolic blood flow velocity(Vp),average blood flow velocity(Vm)),oxidative stress(superoxide dismutase(SOD),advanced oxidized protein product(AOPP),malondialdehyde(MDA))before and after treatment were compared between the two groups.And related serum factors(nerve growth factor(NGF),neuron-specific enolase(NSE),neurotrophic factor(NTF))levels.Result:After treatment,the level of cerebral hemorheology in observation group was higher than that in control group(P<0.05).The total clinical effective rate of the two groups was significantly different(87.50%vs 75.00%,P<0.05).After treatment,the increase of SOD level in observation group was greater than that in control group,and the decrease of AOPP and MDA levels was smaller than that in control group(P<0.05).After treatment,NTF levels in observation group were higher than those in control group,NSE and NGF levels were lower than those in control group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(16.25%vs 8.75%,P>0.05).Conclusion:Troxrutin brain protein hydrolysate combined with niergoline is effective in the treatment of AIS,which can effectively improve cerebral hemorheology,regulate oxidative stress response in vivo,repair neurons,and has high safety.
