房水细胞因子与糖尿病合并白内障第二眼手术时机的相关性分析

摘　　要：目的探讨房水细胞因子与糖尿病合并白内障手术时机的相关性。方法选择2020年6月至2022年12月双眼先后完成白内障手术的糖尿病患者(46例,92眼),实施白内障手术前分别留取术眼房水标本。另选择年龄相匹配的白内障手术患者为对照组(12例,12眼),留取房水标本,通过CBA技术检测血管内皮生长因子(VEGF)、单核细胞趋化蛋白-1(MCP-1)、细胞间黏附分子(ICAM)、干扰素-γ(IFN-γ)和肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)、IL-8、IL-10。比较糖尿病患者第一只眼(A组)与第二只眼(B组)及正常对照组间房水细胞因子的差异,并比较各亚组间(根据第二只眼手术时间分为:≤2周18例;4~8周20例;12~16周8例)房水细胞因子的表达水平差异,分析第二眼白内障手术时机对房水细胞因子的影响。结果双眼手术时间间隔在2周内,与对照组比较,B组IL-8、VEGF、ICAM及IFN-γ均升高,差异有统计学意义(P<0.05);双眼手术时间间隔在4~8周时:与正常对照组比较,B组IL-8、MCP-1、ICAM及IFN-γ值差异有统计学意义(P<0.05);双眼手术时间间隔12周时,A组TNF-α与正常组比较差异有统计学意义(P<0.05),B组VEGF低于A组,差异有统计学意义(P<0.05)。结论白内障超声乳化吸除术后眼内炎症因子水平随时间推移而变化,适当延长双眼手术时间间隔(>12周)可减轻白内障手术时眼部炎症反应,减轻患者术后不适。Objective To investigate the correlation between aqueous humor cytokines and the timing of cataract surgery in diabetes patients.Methods From June 2020 to December 2022,we collected the aqueous humor samples of diabetes patients(46 cases,92 eyes)who had completed cataract surgery in both eyes.Before cataract surgery,we collected the aqueous humor samples of the patients who had undergone the first cataract surgery as the control group(12 cases,12 eyes).The aqueous humor samples were collected to detect vascular endothelial growth factor(VEGF),monocyte chemoattractant protein-1(MCP-1),intercellular adhesion molecule(ICAM),interferon-γ(IFN-γ),tumor necrosis factor-α(TNF-α),interleukin-6(IL-6),IL-8,and IL-10 by CBA technology.Compared the differences in aqueous humor cytokines between the first eye(group A)and the second eye(group B)of the same patient,as well as between the normal control group,and compared the subgroups(grouped according to the surgery time of the second eye,≤2 weeks,18 cases;20 cases at 4~8 weeks;8 cases at 12~16 weeks)to analyze the differential expression levels of aqueous humor cytokines and the impact of the timing of second eye cataract surgery on aqueous humor cytokines.Results Within a 2-week interval between bilateral surgeries,compared with the control group,group B showed an increase in IL-8,VEGF,ICAM,and IFN-γ,with statistically significant differences(P<0.05).When the interval between binocular surgery is 4~8 weeks,there was a statistically significant difference in IL-8,MCP-1,ICAM,and IFN-γvalues between group B and the normal control group.When the interval between binocular surgeries was 12 weeks,there was a statistically significant difference in TNF-αlevels between group A and the normal group(P<0.05),while VEGF levels in group B were lower than those in group A(P<0.05).Conclusion The levels of intraocular inflammatory factors change over time after phacoemulsification for cataracts.Prolonging the interval between bilateral surgeries(>12 weeks)appropriately can reduce th
