甘油三酯血糖指数与脑出血后远隔弥散高信号的相关性分析

摘　　要：目的探讨甘油三酯血糖指数(TyG指数)与脑出血(ICH)后远隔弥散加权成像(DWI)高信号病灶的相关性。方法选择ICH患者187例,收集患者基本信息、血脂、血糖、TyG指数及其他相关指标,结合影像学资料,根据是否发现远隔DWI高信号将患者分成DWI高信号组39例和无DWI高信号组148例;比较两组一般临床特征,采用Logistic回归模型评估两者的相关性。结果共纳入187例ICH患者,其中39例患者合并远隔DWI高信号。DWI高信号组患者的脑小血管病负荷、TyG指数、TyG-BMI指数较无DWI高信号组的患者高,差异有统计学意义(P<0.05),Logistic回归分析显示,脑小血管病负荷、TyG指数是ICH后远隔DWI高信号的影响因素(P<0.05)。通过ROC曲线确定的最佳阈值分别为3分、4.50。结论TyG指数是ICH后远隔DWI高信号的影响因素,胰岛素抵抗可能是ICH患者合并远隔缺血损伤潜在代谢相关指标。Objective To investigate the relationship between triglyceride glycemic index(TyG)index and hyperintense lesions on distal diffusion-weighted imaging(DWI)after intracranial cerebral hemorrhage(ICH).Methods 187 patients with ICH were selected,and their basic information blood lipids,blood glucose,TyG index,and other related indicators were collected.Combined with imaging data,patients were divided into the DWI high signal group(39 cases)and the non DWI high signal group(148 cases)based on whether distant DWI high signal was detected;Compared the general clinical characteristics of two groups and evaluated their correlation using a logistic regression model.Results A total of 187 patients with ICH were enrolled,of whom 39(21.3%)had distal DWI hyperintensities.The cerebral small vessel disease burden,TyG index and TyG-BMI index of patients in the DWI hyperintensity group were higher than those in the non-DWI hyperintensity group,and the difference was statistically significant(P<0.05),and Logistic regression analysis showed that cerebral small vessel disease burden and TyG index were the relevant factors of distal DWI hyperintensities after ICH(P<0.05).The optimal cut-off values determined by the ROC curve were 3 points and 4.50,respectively.Conclusion TyG index is an independent risk factor for distal DWI hyperintensities after ICH,and insulin resistance may be a potential metabolic index in patients with ICH complicated with distal ischemic injury.
