不同术式对近视患者中央角膜内皮细胞及角膜生物力学的影响

摘　　要：目的:探讨不同术式对近视患者中央角膜内皮细胞及角膜生物力学的影响。方法:选取2018年6月—2023年3月漯河市中医院眼科需行眼科手术治疗的近视患者67例,根据手术方法将其分为SBK组(47例)和LASEK组(20例)。LASEK组以LASEK术式进行治疗,SBK组以SBK术式进行治疗,比较两组各项临床指标。结果:SBK组术后1、7 d疼痛指数低于LASEK组(P<0.05);两组术后1个月疼痛指数比较,差异无统计学意义(P>0.05)。两组术后6个月ECD、CV、6A水平较术前降低(P<0.05);两组术前及术后6个月ECD、CV、6A水平组间比较,差异无统计学意义(P>0.05)。两组术后7 d AT1、HC-radius水平较术前降低,DA水平术前升高,且SBK组AT1水平高于LASEK组,DA水平低于LASEK组(P<0.05);两组治疗前后HC-radius水平组间比较,差异无统计学意义(P>0.05)。两组随访期间并发症总发生率比较,差异无统计学意义(P>0.05)。结论:近视患者以SBK进行治疗能够有效减轻患者术后疼痛程度,减少对于角膜生物力学的影响,对于中央角膜内皮细胞的影响与LASEK术式相当,且安全性良好。Objective:To investigate the effects of different surgical methods on central corneal endothelial cells and corneal biomechanics in patients with myopia.Methods:A total of 67 patients with myopia who needed ophthalmic surgery in the Department of Ophthalmology,Luohe Hospital of Traditional Chinese Medicine from June 2018 to March 2023 were selected and divided into SBK group(47 patients)and LASEK group(20 patients)according to the surgical method.The LASEK group was treated with LASEK operation,and the SBK group was treated with SBK operation.The clinical indexes of the two groups were compared.Results:The pain index of SBK group was lower than that of LASEK group at 1 and 7 days after operation(P<0.05),while there was no significant difference in pain index between the two groups at 1 month after operation(P>0.05).The levels of ECD,CV and 6A in the two groups at 6 months after operation were lower than those before operation(P<0.05),while there was no significant difference in ECD,CV and 6A levels between the two groups before and 6 months after operation(P>0.05).At 7 days after operation,the levels of A_(T1)and HC-radius in the two groups were lower than those before operation,and the level of DA was higher than that before operation,while the level of A_(T1)in SBK group was higher than that in LASEK group,and the level of DA was lower than that in LASEK group(P<0.05),there was no significant difference in HC-radius level between the two groups before and after treatment(P>0.05),there was no significant difference in the total incidence of complications between the two groups during follow-up(P>0.05).Conclusion:The treatment of myopia patients with SBK can effectively reduce the degree of postoperative pain and reduce the impact on corneal biomechanics.Meanwhile,the effect on central corneal endothelial cells is equivalent to that of LASEK,and the safety is good.
