低分子肝素联合内镜治疗肝硬化门静脉血栓患者的效果

摘　　要：目的:探究低分子肝素联合内镜治疗对肝硬化门静脉血栓患者排胶出血的影响。方法:选取2023年6月—2024年6月我院接受治疗的肝硬化门静脉血栓患者86例,运用抽签法平均分为参照组和研究组,每组43例。参照组应用内镜下氰基丙烯酸酯注射治疗,研究组采用低分子肝素联合内镜下氰基丙烯酸酯注射治疗。比较两组排胶出血发生率、门静脉再通率、出血及其他不良事件、肝功能。结果:研究组治疗6个月排胶出血发生率低于参照组(P<0.05);研究组门静脉总再通率高于参照组(P<0.05);两组患者出血及其他不良事件发生率比较,差异无统计学意义(P>0.05);研究组治疗6个月Child-Pugh评分低于参照组(P<0.05)。结论:肝硬化门静脉血栓患者应用低分子肝素联合内镜治疗能降低排胶出血发生率,提高门静脉总再通率,改善肝功能,且不会增加出血及其他不良事件发生率,治疗效果显著。Objective:To explore the effect of low molecular weight heparin combined with endoscopic therapy on bleeding in patients with cirrhotic portal vein thrombosis.Methods:From June 2023 to June 2024,86 patients with cirrhosis and portal vein thrombosis who were treated in our hospital were selected.They were divided into reference group and study group by drawing lots,with 43 patients in each group.The reference group was treated with endoscopic cyanoacrylate injection,and the study group was treated with low molecular weight heparin combined with endoscopic cyanoacrylate injection.The incidence of bleeding,portal vein recanalization rate,bleeding and other adverse events,and liver function were compared between the two groups.Results:The incidence of glue bleeding in the study group was lower than that in the reference group at 6 months after treatment(P<0.05).The total recanalization rate of portal vein in the study group was higher than that in the reference group(P<0.05).There was no significant difference in the incidence of bleeding and other adverse events between the two groups(P>0.05).The Child-Pugh score of the study group was lower than that of the reference group at 6 months of treatment(P<0.05).Conclusion:The application of low molecular weight heparin combined with endoscopy in patients with cirrhotic portal vein thrombosis can reduce the incidence of bleeding,improve the total recanalization rate of portal vein,improve liver function,and will not increase the incidence of bleeding and other adverse events.
