3D腹腔镜下右半结肠CME+D3根治术尾侧入路对比中间入路临床疗效分析

摘　　要：探讨3D腹腔镜下右半结肠CME+D3根治术尾侧入路对比中间入路临床疗效。选取2021年3月—2024年4月在贵州省人民医院确诊为盲肠癌、升结肠癌、结肠肝曲癌,并行完全腹腔镜下尾侧入路及中间入路右半结肠癌CME+D3根治术的112例患者为研究对象,采用回顾性分析方法,其中尾侧入路组54例,中间入路组58例,对比两组患者临床指标。研究结果显示,尾侧入路组手术时间、术中出血量均显著小于中间入路组(P<0.05);两组排气时间、住院时间、拔管时间、主干血管根部淋巴结、系膜完整性、术后并发症发生率无显著性差异(P>0.05)。研究发现,尾侧入路较中间入路更容易进入右侧Toldts平面,尾侧入路可以缩短手术时间,减少术中出血量,其学习曲线更短。To explore the clinical efficacy of the caudal approach compared to the intermediate approach in 3D laparoscopic right colon CME+D3 radical surgery.112 patients diagnosed with colorectal cancer,ascending colon cancer,and hepatic flexure carcinoma at Guizhou Provincial People's Hospital from March 2021 to April 2024,who underwent complete laparoscopic caudal and intermediate approaches for right hemicolon cancer CME+D3 radical surgery,were selected as the study subjects.A retrospective analysis method was used,with 54 patients in the caudal approach group and 58 patients in the intermediate approach group.Clinical indicators were compared between the two groups of patients.The research results showed that the operation time and intraoperative blood loss in the caudal approach group were significantly lower than those in the intermediate approach group(<0.05);There was no significant difference(>0.05)in exhaust time,hospitalization time,extubation time,root lymph nodes of main blood vessels,mesangial integrity,and incidence of postoperative complications between the two groups.Research has found that the caudal approach is easier to enter the right Toldts plane than the middle approach.The caudal approach can shorten surgical time,reduce intraoperative bleeding,and have a shorter learning curve.
