摘 要:目的:探讨大脑凸面脑膜瘤瘤周静脉分型与术后瘤床出血的相关性。方法:选取2021年6月—2023年12月我院行开颅大脑凸面脑膜瘤切除术治疗的66例大脑凸面脑膜瘤患者为研究对象,所有患者术前经磁共振颅脑静脉成像(MRV)与磁共振成像(MRI)平扫及增强检查确定瘤周静脉分型;统计不同瘤周静脉分型患者术后瘤床出血情况,分析大脑凸面脑膜瘤瘤周静脉分型与大脑凸面脑膜瘤患者术后瘤床出血的相关性。结果:随访6个月时,66例大脑凸面脑膜瘤患者中9例(13.64%)患者发生术后瘤床出血;57例(86.36%)患者未发生术后瘤床出血;发生组性别、体质量指数、年龄、临床症状、肿瘤直径、术前四肢肌力、瘤周水肿、肿瘤位置、肿瘤病理WHO分级、术中肿瘤蛛网膜界面情况与未发生组比较,差异无统计学意义(P>0.05);发生组肿瘤Simpson切除程度与未发生组比较,差异有统计学意义(P<0.05);发生组大脑凸面脑膜瘤瘤周静脉分型与未发生组比较,差异有统计学意义(P<0.05);经logistic回归分析显示,肿瘤Simpson切除程度、瘤周静脉分型Ⅰ型、瘤周静脉分型Ⅳ型与大脑凸面脑膜瘤患者术后瘤床出血有关(OR>1,P<0.05)。结论:大脑凸面脑膜瘤瘤周静脉分型与大脑凸面脑膜瘤患者术后瘤床出血有关。Objective:To explore the correlation between peritumoral venous classification and postoperative tumor bed bleeding in cerebral convex meningioma.Methods:A total of 66 patients with cerebral convex meningioma who underwent craniotomy at the Third Hospital of Nanchang from June 2021 to December 2023 were selected as the study subjects.All patients underwent preoperative magnetic resonance cerebral venous imaging(MRV),magnetic resonance imaging(MRI)plain scan,and enhanced examination to determine the classification of peritumoral veins.The postoperative tumor bed bleeding in patients with different peritumoral vein types was counted,and the correlation between peritumoral vein types and postoperative hemorrhage of tumor bed in patients with cerebral convex meningioma was analyzed.Results:During a 6-month follow-up,9 out of 66 patients with cerebral convex meningioma experienced postoperative tumor bed bleeding,accounting for 13.64%.57 patients did not experience postoperative tumor bed bleeding,accounting for 86.36%.There was no statistical significant difference in the gender,body mass index,age,clinical symptoms,tumor diameter,preoperative limb muscle strength,peritumoral edema,tumor location,tumor pathology WHO grading,and intraoperative tumor arachnoid interface between the occurrence group and the non occurrence group(P>0.05).There was a statistical significant difference in the degree of Simpson resection between the occurrence group and the non occurrence group(P<0.05).There was a statistical significant difference in the classification of peritumoral veins between the occurrence group and the non occurrence group of cerebral convex meningioma(P<0.05).Through logistic regression analysis,the results showed that the degree of tumor Simpson resection,peritumoral venous classification typeⅠ,and peritumoral venous classification typeⅣwere related to postoperative tumor bed bleeding in patients with cerebral convex meningioma(OR>1,P<0.05).Conclusion:The peritumoral venous classification of cerebral convex men
