国医大师张震胆病治肝思想探析

摘　　要：中医基础理论认为,肝胆在解剖结构、生理功能、经脉络属及病理改变等方面关系密切。生理上,胆喜温和宁谧,肝之余气化而为胆;肝胆相互为用,共同促进消化、调节精神情志、和谐脏腑气机;肝胆经脉络属,加强肝胆联系。病理上相互影响,常胆病及肝、肝病及胆或肝胆同病。在肝胆关系中,肝为主导,胆为辅从,故胆病应责之于肝,其治亦在肝。国医大师张震教授提出以“一体两翼、疏调气机”为基础治疗思路,诊疗中结合“两态三三构型规律”进行辨证,采用对病、对证、对症三对应的病证结合诊疗模式,以期丰富肝胆理论内涵和胆病的治疗思路。In the basic theory of traditional Chinese medicine,it is believed that there is a close relationship between the Liver and Gallbladder in anatomical structure,physiological function,meridians,and pathological changes.Physiologically,the Gallbladder likes mildness and tranquility,and the residual Qi of the Liver is transformed into Gallbladder.The Liver and Gallbladder support each other to promote digestion,regulate the spirit and mood,and harmonize the Qi activity of the internal organs.The Liver and Gallbladder meridians communicate with each other to strengthen the connection between the Liver and the Gallbladder.Pathologically,disease of both interact with each other,namely,the Liver disease and the Gallbladder disease can affect the other or both get injured at the same time.In the relationship between Liver and Gallbladder,the Liver is dominating while the Gallbladder is the auxiliary one,so treating Gallbladder disease should treating the Liver first.In modern medicine,the treatment of Gallbladder disease from the perspective of Liver is also scientific.Prof.Zhang Zhen,TCM national master,proposed"based on the concept of one body with two wings the Qi activity being dredged and regulated"in clinical treatment.With the"two states,three and three configurations"in diagnosis the new treatment mode of considering the disease,the syndrome and the symptom at the same time is established,in order to enrich the connotation of Liver and Gallbladder theory and the treatment idea of the Gallbladder disease.
