摘 要:目的探究结直肠癌伴慢性肾脏病(CKD)患者外周血DKK1、Klotho的表达,分析其与骨代谢的关系。方法选取78例结直肠癌伴CKD患者作为观察组,同期纳入健康体检者68例为对照组,收集一般资料,并行外周血DKK1、Klotho水平和骨代谢指标检测。采用Pearson法分析外周血DKK1、Klotho水平与骨代谢指标的相关性。结果2组一般资料差异无统计学意义(P>0.05)。观察组外周血DKK1表达和骨代谢指标磷、25羟维生素D[25(OH)D]、全段甲状旁腺素(iPTH)、β胶原羧基端交联肽(β-CTX)、Ⅰ型胶原氨基端前肽(PINP)、骨钙素(BGP)、骨碱性磷酸酶(BAP)表达均高于对照组,Klotho表达低于对照组(P<0.05)。经Pearson法分析发现骨代谢指标磷、25(OH)D、iPTH、β-CTX、PINP、BGP、BAP与DKK1呈正相关性,与Klotho呈负相关性。结论结直肠癌伴CKD患者外周血DKK1异常偏高,Klotho表达异常偏低,DKK1表达可能参与骨代谢,Klotho表达可能抑制骨代谢。Objective To explore the expression of DKK1 and Klotho in the peripheral blood of patients with colorectal cancer and chronic kidney disease(CKD)and analyze its relationship with bone metabolism.Methods 78 patients with colorectal cancer and CKD were selected as the observation group,and 68 healthy people who underwent physical examination during the same period were selected as the control group.General data were collected,and peripheral blood DKK1 and Klotho levels and bone metabolism index detection were measured.Pearson method was used to analyze the correlation between peripheral blood DKK1 and Klotho levels and bone metabolism indexes.Results There was no statistically significant difference in general information between the 2 groups(P>0.05).The expression of DKK1 in peripheral blood and bone metabolism indexes of phosphorus,25-hydroxyvitamin D[25(OH)D],intact parathyroid hormone(iPTH),β-collagen carboxy-terminal cross-linked peptide(β-CTX),and amino-terminal propeptide(PINP),osteocalcin(BGP),and bone alkaline phosphatase(BAP)were higher than those in the control group,while the expression of Klotho was lower than that in the control group(P<0.05).The Pearson method analysis showed that the bone metabolism indicators phosphorus,25(OH)D,iPTH,β-CTX,PINP,BGP,BAP were positively correlated with DKK1 and negatively correlated with Klotho.Conclusion DKK1 in the peripheral blood of patients with colorectal cancer and CKD is abnormally high,and Klotho expression is abnormally low.DKK1 expression may be involved in bone metabolism,and Klotho expression may inhibit bone metabolism.
