摘 要:目的采用生物信息学方法探索头颈部鳞状细胞癌(HNSCC)组织中的缺氧相关基因(HAGs),并探讨其与预后的关系。方法基于TCGA数据库HNSCC患者的遗传和临床信息,采用limma法对癌和癌旁数据进行差异分析。单因素Cox回归分析筛选出与HNSCC患者生存预后显著相关的HAGs,Lasso回归和多因素Cox回归分析构建预后模型,在E-MTAB-8588数据集中进行验证。构建列线图并评估预测价值。采用GO和KEGG富集分析来研究风险模型的分子和生物学功能;进行免疫相关性分析。结果从MSigDB数据库获得200个缺氧相关基因,结合TCGA数据库中头颈部鳞癌数据,通过差异分析共得到57个HAGs差异表达缺氧相关基因。使用Lasso回归筛选出9个具有预后意义的缺氧相关基因,并建立了头颈部鳞癌缺氧相关基因预后模型。Kaplan-Meier分析显示,在训练集、验证集中低危组患者OS均优于高危组患者(P<0.01)。ROC曲线均显示该风险模型有较好的预测效能。多因素Cox回归分析显示,风险评分、肿瘤分期、年龄与HNSCC患者的OS有关(P<0.05)。结论该研究筛选出的9个缺氧相关基因可能是HNSCC预后的潜在标志物,以此构建的列线图预测HNSCC患者预后效能较好。Objective To explore hypoxia-associated genes(HAGs)in head and neck squamous cell carcinoma(HNSCC)by bioinformatics methods,and to explore the relationship between HAGS and prognosis,providing a new therapeutic target for HNSCC patients.Methods The TCGA database provided genetic and clinical information on HNSCC patients.Differential expressed genes between cancer and adjacent tissues were analyzed by limma method.HAGs significantly correlated with prognosis in patients with HNSCC were identified by univariate.A prognostic model was established through Lasso regression and multivariate Cox regression analyses.This risk model was validated in E-MTAB-8588 datasets.A nomogram was constructed and assessed the predictive value.GO,and KEGG enrichment analyses was used to investigate the molecular and biological functions of the risk model.Immune status and metabolic pathways were analyzed.Results 200 hypoxia-associated genes were obtained from the MSigDB database.Combining with HNSCC cancer data in the TCGA database,57 HAGs were screened through differential analysis.Nine prognosis-related genes were screened out by Lasso regression and the prognostic model with hypoxia-associated genes of HNSCC was established.Kaplan-Meier survival curve analysis showed that the overall survival rate of the low risk group was superior to the high risk group in both the train set and the test set(P<0.01).ROC curve verified that the reliability of the prediction model was good.Univariate COX regression analysis showed that risk score,tummor stage and age were significantly correlated with overall survival of HNSCC patients(P<0.05).Conclusion The prognostic risk score model constructed by 9 genes relevant to hypoxia in this study may be a potential marker of prognosis of the patients with HNSCC,and the nomogram constructed by this model is more effective in predicting the prognosis of patients with HNSCC.
