血清肿瘤异常蛋白表达与直肠癌新辅助化疗患者临床病理特征及其化疗效果的关系

摘　　要：目的分析血清肿瘤异常蛋白(TAP)表达与直肠癌新辅助化疗患者临床病理特征及其化疗效果的关系。方法收集80例直肠癌新辅助化疗患者的临床资料进行研究。依照患者化疗效果分为良好组(58例)和不良组(22例)。比较不同疗效患者的年龄、性别、组织分化程度、组织学类型;分析TAP表达水平与直肠癌临床病理特征的关系;分析TAP表达水平与直肠癌患者化疗效果的关系及其对化疗效果的预测价值。结果两组年龄、性别、组织分化程度、组织学类型比较差异无统计学意义(P>0.05)。不同年龄、性别、组织学类型患者的TAP表达水平差异不显著(P>0.05),不同组织分化程度患者TAP表达水平差异显著,低未分化患者的TAP表达水平高于高、中分化患者(t=2.755,P=0.007)。不良组血清TAP表达水平[(222.65±60.34)μm^(2)]高于良好组[(184.47±52.11)μm^(2)],差异显著(t=2.434,P=0.017)。血清TAP表达水平对直肠癌患者化疗效果有良好的预测价值,受试者操作特征曲线下面积(AUC)为0.719,灵敏度为0.707,特异度为0.773。TAP的预测阈值为205.805μm^(2)。结论直肠癌患者血清TAP表达水平可能与组织分化程度、新辅助化疗效果不良有关,TAP的检测可作为临床上直肠癌新辅助化疗效果的评估指标。Objective To analyze the relationship between the expression of serum abnormal tumor protein(TAP)and the clinicopathologic features and chemotherapy effect of neoadjuvant chemotherapy for rectal cancer.Methods The clinical data of 80 patients with neoadjuvant chemotherapy for rectal cancer were collected and studied.Patients were divided into good group(58 cases)and bad group(22 cases)according to the effect of chemotherapy.The age,sex,degree of tissue differentiation and histological type of patients with different therapeutic effects were compared.The relationship between TAP expression level and clinicopathological features of rectal cancer patients was analyzed.To analyze the relationship between TAP expression level and chemotherapy effect in rectal cancer patients and its predictive value for chemotherapy effect.Results There was no significant difference in age,sex,degree of tissue differentiation and histological type between the 2 groups(P>0.05).There were no significant differences in the expression level of TAP in patients with different age,gender and histological type(P>0.05),but there were significant differences in the expression level of TAP in patients with different degrees of tissue differentiation.The expression level of TAP in low undifferentiated patients was higher than that in high differentiated patients(t=2.755,P=0.007).The serum TAP expression level in the poor group[(222.65±60.34)μm^(2)]was higher than that in the good group[(184.47±52.11)μm^(2)],and the difference was significant(t=2.434,P=0.017).Serum TAP expression level has a good predictive value for chemotherapy effect in rectal cancer patients,and the area under the subject operating characteristic curve(AUC)is 0.719,the sensitivity is 0.707,and the specificity is 0.773.The predictive threshold of TAP is 205.805μm^(2).Conclusion The level of serum TAP expression in patients with rectal cancer may be related to the degree of tissue differentiation and the poor effect of neoadjuvant chemotherapy,and the detection of TAP can b
