摘 要:目的探究循环肿瘤细胞(CTCs)以及全身免疫炎症指数(SII)在乳腺癌患者化疗疗效评估中的应用价值。方法收集90例乳腺癌患者临床资料进行回顾性分析。依照化疗疗效的不同将92例患者分为疗效良好的良好组(n=71例)和疗效不佳的不良组(n=21例)。比较两组基线资料、CTCs表达以及SII;采用受试者操作特征曲线下面积(AUC)分析CTCs以及SII对乳腺癌化疗疗效的预测效能。结果不同疗效患者的年龄、体质量指数、文化程度、Ki-67、组织学分级、淋巴结分期、肿块大小比较,差异无统计学意义(t=0.288,P=0.774、t=0.638,P=0.525、χ^(2)=0.226,P=0.634、χ^(2)=3.024,P=0.082、χ^(2)=0.878,P=0.349、χ^(2)=0.131,P=0.717、χ^(2)=0.269,P=0.604)。不良组CTCs阳性表达率、SII均高于良好组,差异有统计学意义(χ^(2)=9.458,P=0.002、t=3.051,P=0.003)。CTCs、SII对乳腺癌患者化疗疗效有良好的预测效能,受试者操作特征曲线下面积(AUC)分别为0.904、0.886,灵敏度分别为0.876、0.902,特异度分别为0.878、0.802,SII的预测阈值为489.622。与单一指标相比,二者联合对患者化疗疗效的预测价值较大,AUC最大为0.912,灵敏度、特异度分别为0.908、0.886。结论CTCs及SII在乳腺癌患者化疗疗效评估中有良好的应用价值,二者联合的预测效能最佳,可作为乳腺癌患者的化疗疗效评估因子。Objective To explore the application value of circulating tumor cells(CTCs)and systemic immunoinflammatory index(SII)in the evaluation of chemotherapy efficacy in breast cancer patients.Methods Clinical data of 90 patients with breast cancer were retrospectively analyzed.According to the different chemotherapy effects,92 patients were divided into good group(n=71 cases)and bad group(n=21 cases).Baseline data,CTCs expression and SII were compared between the 2 groups.The area under the receiver operating characteristic curve(AUC)was used to evaluate the predictive efficacy of CTCs and SII in breast cancer chemotherapy.Results There was no significant difference in age,body mass index,education level,Ki-67,histological grade,lymph node stage and tumor size(t=0.288,P=0.774,t=0.638,P=0.525,χ^(2)=0.226,P=0.634,χ^(2)=3.024,P=0.082,χ^(2)=0.878,P=0.349,2=0.131,P=0.717,χ^(2)=0.269,P=0.604).The positive expression rate of CTCs and SII in the poor group were higher than those in the good group,and the difference was statistically significant(χ^(2)=9.458,P=0.002,t=3.051,P=0.003).CTCs and SII showed good predictive efficacy for chemotherapy in breast cancer patients.AUC was 0.904 and 0.886,sensitivity was 0.876 and 0.902,and specificity was 0.878 and 0.802,respectively.The predictive threshold of SII was 489.622.Compared with a single index,the combination of the 2 methods had greater predictive value for chemotherapy efficacy,with the maximum AUC of 0.912,sensitivity and specificity of 0.908 and 0.886,respectively.Conclusion CTCs and SII have good application value in the evaluation of chemotherapy efficacy in breast cancer patients.The combination of CTCS and SII has the best predictive efficacy and can be used as the evaluation factor of chemotherapy efficacy in breast cancer patients.
