摘　　要：目的分析内镜下黏膜下剥离术(ESD)治疗胃早癌对患者手术相关指标、Livin、EGF的影响。方法选取96例胃早癌患者,采用随机数字表法将其分为2组,对照组48例采用腹腔镜微创手术治疗、观察组48例采用ESD治疗。比较2组患者的近期疗效、手术相关指标、血清Livin、EGF水平、术中及术后并发症发生情况。结果观察组总有效率高于对照组,差异有统计学意义(χ^(2)=4.444,P=0.035)。观察组手术时间为(72.63±8.14)min、出血量为(139.65±46.52)ml、住院时间为(7.55±1.50)d,均低于对照组的(76.12±8.25)min、(161.28±49.63)ml、(8.63±1.55)d,差异均有统计学意义(P<0.05)。治疗前,2组Livin、EGF水平比较,差异无统计学意义(t=0.536,P=0.593、t=0.125,P=0.901);治疗后,2组Livin、EGF水平均降低,且观察组低于对照组,差异均有统计学意义(P<0.05)。与对照组16.67%的并发症总发生率相比,观察组(2.08%)更低(χ^(2)=4.414,P=0.036)。结论ESD近期疗效好,可减少出血量,缩短手术时间及住院时间,降低胃早癌患者血清Livin、EGF水平,且相对安全。Objective To analyze the effects of endoscopic submucosal dissection(ESD)on surgical indicators,Livin,EGF in patients with early gastric cancer.Methods 96 patients with early gastric cancer were selected and divided into 2 groups by random number table method,48 patients in the control group received laparoscopic minimally invasive surgery and 48 patients in the observation group received ESD.The short-term efficacy,operation-related indexes,serum Livin,EGF levels,intraoperative and postoperative complications were compared between the 2 groups.Results The total effective rate of the observation group was higher than that of the control group,and the differences were statistically significant(χ^(2)=4.444,P=0.035).The operation time of the observation group was(72.63±8.14)min,the amount of blood loss was(139.65±46.52)ml,and the length of hospitalization was(7.55±1.50)d,which were lower than that of the control group(76.12±8.25)min,(161.28±49.63)ml,(8.63±1.55)d.The difference was statistically significant(P<0.05).Before treatment,there was no significant difference in Livin and EGF levels between the two groups(t=0.536,P=0.593,t=0.125,P=0.901).After treatment,the levels of Livin and EGF in both groups were decreased,and the observation group was lower than the control group,with statistical significance(P<0.05).The overall complication rate was lower in the observation group(2.08%)compared with 16.67%in the control group(χ^(2)=4.414,P=0.036).Conclusion ESD has good short-term efficacy,and can reduce blood loss,shorten operation time and hospital stay,and reduce serum Livin and EGF levels for patients with early gastric cancer,and it is relatively safe.
