摘 要:目的 探讨血清中的肿瘤坏死因子-α (TNF-α)和白细胞介素-6(IL-6),以及血脂中的低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)、甘油三脂(TG)与牙周病间的相关性。方法 单纯牙周炎无冠心病患者35例(CP组),单纯冠心病无牙周炎患者35例(CHD组),伴有冠心病的牙周炎患者20例(CP+CHD组),健康对照组 45例(H 组)。采集所有人员的血清和血脂,进行牙周临床指标的检查。结果 四组检验对象的年龄、性别和体重指标资料之间的差异没有统计学意义(P>0.05)。CP 组、CP+CHD 组与 H 组之间的牙周探诊深度和出血指数之间差异有统计学意义(P<0.05)。TNF-α 和 IL-6 与牙周探诊深度和出血指数之间呈现正相关(00.05)。牙周基础治疗后,TNF-α、IL-6、HDL-C和TG值均下降,LDL-C值升高,与治疗前相比TNF-α和 IL-6差异具有统计学意义(P<0.05),而 TG、HDL-C 和 LDL-C 差异无统计学意义(P>0.05)。结论 牙周炎中的 TNF-α 和 IL-6 炎性因子会导致牙周组织的破坏,增加冠心病的风险。Objective To investigate the correlation between tumor necrosis factor- α(TNF-α),interleukin-6(IL-6),low density lipoprotein cholesterol (LDL-C),high density lipoprotein cholesterol (HDL-C) and triglycerides (TG) in serum and periodontal disease.Methods There were 35 patients with simple periodontitis without coronary heart disease (CP group),35 patients with simple coronary heart disease without periodontitis (CHD group),20 patients with periodontitis accompanied by coronary heart disease (CP+CHD group),and 45 healthy control group (H group).The serum and blood lipids of all participants were collected,and the periodontal clinical indicators were examined.Results There were no significant differences in age,sex and body weight among the four groups (P>0.05).There were significant differences in periodontal probing depth and bleeding index among CP group,CP+CHD group and H group (P<0.05).TNF- α and IL-6 were positively correlated with periodontal depth and bleeding index (00.05).After periodontal basic treatment,TNF-α,IL-6,HDL-C and TG values all decreased,and LDL-C values increased,with statistically significant differences in TNF-α and IL-6 compared with before treatment (P<0.05),while there was no statistically significant difference in TG,HDL-C and LDL-C (P>0.05).Conclusion TNF- α and IL-6 inflammatory factors in periodontitis can lead to the destruction of periodontal tissue and increase the risk of coronary heart disease.
