摘 要:目的研究动态增强磁共振成像(dynamic contrast enhancement magnetic resonance imaging,DCE-MRI)与弥散加权成像(diffusion weighted imaging,DWI)应用在乳腺肿瘤良恶性病变鉴别诊断的效果及准确性。方法选取2020年1月—2023年1月南京大学医学院附属盐城第一医院收治的80例乳腺肿瘤患者为研究对象,所有患者均接受DCE-MRI扫描及DWI扫描,以病理检查结果为金标准,分析两种方法单独应用及联合应用的诊断结果及诊断效能。结果病理检查结果诊断良性64例,恶性16例;DCE-MRI检出良性55例,恶性25例;DWI检出良性61例,恶性19例;DCE-MRI联合DWI检出良性67例,恶性13例。DCE-MRI联合DWI诊断的特异度为96.88%(62/64)、准确度为91.25%(73/80),均高于DCE-MRI诊断的75.00%(48/64)、71.25%(57/80)及DWI诊断的82.81%(53/64)、76.25%(61/80),差异均有统计学意义(χ^(2)=12.267、10.668,P均<0.05)。3种检测方法的灵敏度比较,差异无统计学意义(P>0.05)。结论乳腺肿瘤良恶性病变的鉴别诊断中应用DCE-MRI联合DWI的诊断效能更高。Objective To study the effect and accuracy of dynamic contrast enhancement magnetic resonance imaging(DCE-MRI)and diffusion weighted imaging(DWI)in the differential diagnosis of benign and malignant breast tumors.Methods From January 2020 to January 2023,a total of 80 patients with breast tumors admitted to the Yancheng First Hospital Affiliated of Nanjing University Medical College were selected as the research objects.All patients underwent DCE-MRI scan and DWI scan.The pathological examination results were used as the gold standard to analyze the diagnostic results and diagnostic efficacy of the two methods alone and in combination.Results The results of pathological examination diagnosis that 64 cases were benign and 16 cases were malignant.DCE-MRI detected 55 cases of benign and 25 cases of malignant;DWI detected 61 cases of benign and 19 cases of malignant;DCE-MRI combined with DWI detected 67 cases of benign and 13 cases of malignant.The specificity and accuracy of DCE-MRI combined with DWI were 96.88%(62/64)and 91.25%(73/80),which were higher than 75.00%(48/64)and 71.25%(57/80)of DCE-MRI and 82.81%(53/64)and 76.25%(61/80)of DWI,the differences were statistically significant(χ^(2)=12.267,10.668,both P<0.05).There was no significant difference in the sensitivity of the three detection methods(P>0.05).Conclusion The application of DCE-MRI combined with DWI in the differential diagnosis of benign and malignant breast tumors has higher diagnostic efficiency.
