摘  要:目的探讨多时相CT血管成像(multi-phase CT angiography,mCTA)在诊断缺血性脑卒中疾病中的应用价值。方法选取2020年1月—2023年6月句容市人民医院收治的200例疑似缺血性脑卒中患者为研究对象,均进行单时相CT血管成像(single-phase CT angiography,sCTA)与mCTA检查,以临床综合诊断结果为金标准,比较sCTA、mCTA诊断血管狭窄的诊断效能及诊断符合率。结果200例疑似缺血性脑卒中患者中,临床综合诊断确诊124例患者为脑血管狭窄/闭塞。mCTA诊断的灵敏度为96.77%(120/124)、特异度为98.68%(75/76)、准确度为97.50%(195/200),均高于sCTA的89.52%(111/124)、86.84%(66/76)、88.50%(177/200),差异均有统计学意义(χ^(2)=5.115、7.938、12.442,P均<0.05)。mCTA诊断颅内动脉近段闭塞、颅内动脉远端闭塞的符合率均高于sCTA,差异均有统计学意义(P均<0.05)。两种诊断方法诊断颅内动脉近段狭窄、颅内动脉远段狭窄的符合率比较,差异均无统计学意义(P均>0.05)。结论临床诊断缺血性脑卒中疾病可采取mCTA技术,其诊断效能较高,同时对颅内动脉闭塞的诊断符合率更高。Objective To explore the application value of multi-phase CT angiography(mCTA)in the diagnosis of ischemic stroke.Methods A total of 200 patients with suspected ischemic stroke admitted to Jurong People's Hospital from January 2020 to June 2023 were selected as the study objects.Single-phase CT angiography(sCTA)and mCTA were performed in all patients,and comprehensive clinical diagnosis results were used as the gold standard.The diagnostic efficacy and diagnostic coincidence rate of sCTA and mCTA in the diagnosis of vascular stenosis were compared.Results Among 200 patients suspected of ischemic stroke,124 patients were diagnosed with cerebrovascular stenosis/occlusion by clinical comprehensive diagnosis.The diagnostic sensitivity of mCTA was 96.77%(120/124),specificity was 98.68%(75/76)and accuracy was 97.50%(195/200),which were higher than 89.52%(111/124),86.84%(66/76)and 88.50%(177/200)of sCTA,and the differences were statistically significant(χ^(2)=5.115,7.938,12.442,all P<0.05).The coincidence rate of mCTA in diagnosis of intracranial artery proximal occlusion and intracranial artery distal occlusion were higher than that of sCTA,and the differences were statistically significant(both P<0.05).There were no significant differences in the coincidence rates of proximal intracranial artery stenosis and distal intracranial artery stenosis between the two diagnostic methods(both P>0.05).Conclusion Clinical diagnosis of ischemic stroke disease can take mCTA technology,its diagnostic efficiency is high,and the diagnostic coincidence rate of intracranial artery occlusion is higher.
