摘  要:目的探讨微生态制剂对乙肝相关慢加急性肝衰竭继发感染患者肝功能、肠道菌群及免疫功能的影响。方法纳入2020年1月至2022年11月新乡市第二人民医院收治的98例乙肝相关慢加急性肝衰竭继发感染患者,以随机数字表法将其分为对照组(常规治疗)和观察组(常规治疗联合微生态制剂),各49例。比较两组的治疗效果。结果观察组的治疗总有效率高于对照组(P<0.05)。治疗后,观察组的谷丙转氨酶(ALT)、谷草转氨酶(AST)、总胆红素(TBIL)水平低于对照组(P<0.05)。治疗后,观察组的乳酸杆菌、双歧杆菌数量高于对照组,变形杆菌、肠球菌数量低于对照组(P<0.05)。治疗后,观察组的CD4^(+)、CD4^(+)/CD8^(+)高于对照组,白细胞介素-6(IL-6)、白细胞介素-1(IL-1)水平低于对照组(P<0.05)。治疗后,观察组的胆碱酯酶(CHE)水平高于对照组,层粘连蛋白(LN)、透明质酸(HA)水平低于对照组(P<0.05)。结论微生态制剂能够有效提高乙肝相关慢加急性肝衰竭继发感染患者临床疗效,调节肠道菌群,改善肝功能、肝纤维化和免疫功能,抑制炎症反应。Objective To investigate the effects of microecological preparation on liver function,intestinal flora and immune function in patients with secondary infection of hepatitis B-related acute-on-chronic liver failure.Methods A total of 98 patients with secondary infection of hepatitis B-related acute-on-chronic liver failure admitted in the Second People's Hospital of Xinxiang from January 2020 to November 2022 were included and divided into control group(routine treatment)and observation group(routine treatment combined with microecological preparation)by random number table method,with 49 cases in each group.The therapeutic effects of the two groups were compared.Results The total effective rate of treatment in the observation group was higher than that of the control group(P<0.05).After treatment,the levels of alanine aminotransferase(ALT),aspartate aminotransferase(AST)and total bilirubin(TBIL)in the observation group were lower than those in the control group(P<0.05).After treatment,the number of Lactobacillus and Bifidobacterium in the observation group were higher than those in the control group,and the number of Proteus and Enterococcus were lower than those in the control group(P<0.05).After treatment,the levels of CD4^(+)and CD4^(+)/CD8^(+)in the observation group were higher than those in the control group,and the levels of interleukin-6(IL-6)and interleukin-1(IL-1)were lower than those in the control group(P<0.05).After treatment,the level of cholinesterase(CHE)in the observation group was higher than that in the control group,and the levels of laminin(LN)and hyaluronic acid(HA)were lower than those in the control group(P<0.05).Conclusion Microecological preparation can effectively improve the clinical efficacy of patients with secondary infection of hepatitis B-related acute-on-chronic liver failure,regulate intestinal flora,improve liver function,liver fibrosis and immune function,and inhibit inflammatory response.
