替奈普酶与阿替普酶联合丁苯酞治疗急性缺血性脑卒中的效果及对神经、炎性因子水平的影响比较

摘　　要：目的比较替奈普酶与阿替普酶联合丁苯酞治疗急性缺血性脑卒中(AIS)的效果及对神经、炎性因子水平的影响。方法选取2021年11月至2024年4月我院收治的64例AIS患者为研究对象,按照随机数字表法将其分为阿替普酶组(32例,阿替普酶+丁苯酞治疗)及替奈普酶组(32例,替奈普酶+丁苯酞治疗)。比较两组的治疗效果。结果治疗后,替奈普酶组的神经生长因子(NGF)水平高于阿替普酶组,神经元特异性烯醇化酶(NSE)、中枢神经特异性蛋白β(S-100β)、C反应蛋白(CRP)、肿瘤坏死因子-α(TNF-α)及白细胞介素-6(IL-6)水平均低于阿替普酶组(P<0.05)。治疗后3个月,替奈普酶组的巴氏指数(BI)评分高于阿替普酶组(P<0.05)。结论替奈普酶与阿替普酶联合丁苯酞治疗AIS的效果相近,但替奈普酶联合丁苯酞治疗对机体神经、炎性因子水平及预后的改善效果更好。Objective To compare the effects of tenecteplase and alteplase combined with butylphthalide in the treatment of acute ischemic stroke(AIS)and they influences on the levels of nerve factors and inflammatory factors.Methods A total of 64 patients with AIS admitted in our hospital from November 2021 to April 2024 were selected as the research objects.According to the random number table method,the patients were divided into alteplase group(32 cases,alteplase+butylphthalide treatment)and tenecteplase group(32 cases,tenecteplase+butylphthalide treatment).The therapeutic effects of the two groups were compared.Results After treatment,the level of nerve growth factor(NGF)in the tenecteplase group was higher than that in the alteplase group,and the levels of neuron-specific enolase(NSE),central nervous system specific proteinβ(S-100β),C-reactive protein(CRP),tumor necrosis factor-α(TNF-α)and interleukin-6(IL-6)were lower than those in the alteplase group(P<0.05).At 3 months after treatment,the Barthel Index(BI)score of the tenecteplase group was higher than that of the alteplase group(P<0.05).Conclusion The tenecteplase and alteplase combined with butylphthalide in the treatment of AIS has similar effect,but the improvement effect of tenecteplase combined with butylphthalide on the levels of nerve factors and inflammatory factors and prognosis is better.
