外周血淋巴细胞亚群水平与直肠癌患者临床病理特征的关系

摘　　要：目的分析外周血淋巴细胞亚群水平与直肠癌患者临床病理特征的关系。方法选取2021年10月—2022年12月广西医科大学第一附属医院结直肠肛门外科收治的直肠癌患者189例作为病例组,选择同期就诊的非恶性肿瘤患者39例作为对照组。比较两组患者外周血淋巴细胞亚群水平及直肠癌患者不同临床病理特征外周血淋巴细胞亚群水平,并分析外周血淋巴细胞亚群水平与直肠癌患者临床病理特征的关系。结果病例组CD3^(+)和CD3^(+)CD8^(+)水平低于对照组,差异有统计学意义(P<0.05);两组CD3^(+)CD4^(+)、CD19^(+)和自然杀伤(NK)细胞水平比较,差异无统计学意义(P>0.05)。直肠癌患者肿瘤最大直径≥5 cm组的CD19^(+)水平低于肿瘤最大直径<5 cm组,差异有统计学意义(P<0.05);肿瘤最大直径≥5 cm组和肿瘤最大直径<5 cm组的CD3^(+)、CD3^(+)CD4^(+)、CD3^(+)CD8^(+)和NK细胞水平比较,差异无统计学意义(P>0.05)。直肠癌患者T3或T4期CD3^(+)水平低于T1或T2期,差异有统计学意义(P<0.05);T3或T4期和T1或T2期的CD3^(+)CD4^(+)、CD3^(+)CD8^(+)、CD19^(+)和NK细胞水平比较,差异无统计学意义(P>0.05)。直肠癌患者淋巴结转移、癌结节、脉管癌栓、神经侵犯和TNM分期不同组的CD3^(+)、CD3^(+)CD4^(+)、CD3^(+)CD8^(+)、CD19^(+)和NK细胞水平比较,差异无统计学意义(P>0.05)。结论直肠癌患者免疫功能下降,其外周血淋巴细胞亚群水平与患者临床病理特征密切相关。Objective To analyze the relationships between the levels of peripheral blood lymphocyte subsets and the clinicopathological features in patients with rectal cancer.Methods A total of 189 patients with rectal cancer admitted to the Department of Colorectal and Anal Surgery in the First Affiliated Hospital of Guangxi Medical University from October 2021 to December 2022 were selected as the case group,while 39 patients with non-malignant tumors who visited the clinic during the same period were selected as the control group.The levels of peripheral blood lymphocyte subsets in the two groups and the levels of peripheral blood lymphocyte subsets in rectal cancer patients with different clinicopathological features were compared,and the relationships between the levels of peripheral blood lymphocyte subsets and the clinicopathological features in patients with rectal cancer were analyzed.Results The levels of CD3^(+)and CD3^(+)CD8^(+)in the case group were lower than those in the control group,and the differences were statistically significant(P<0.05).There were no significant differences in the levels of CD3^(+)CD4^(+),CD19^(+)and natural killer(NK)cells between the two groups(P<0.05).The CD19^(+)levels in rectal cancer patients with the maximum tumor diameter equal or more than 5 cm group were lower than those in rectal cancer patients with the maximum tumor diameter less than 5 cm group,and the differences were statistically significant(P<0.05).There were no significant difference in the levels of CD3^(+),CD3^(+)CD4^(+),CD3^(+)CD8^(+)and NK cells between the maximum tumor diameter equal or more than 5 cm group and the maximum tumor diameter less than 5 cm group in rectal cancer patients(P>0.05).The CD3^(+)levels in T3 or T4 stage in patients with rectal cancer were lower than those in T1 or T2 stage,and the differences were statistically significant(P<0.05).There were no significant differences in the levels of CD3^(+)CD4^(+),CD3^(+)CD8^(+),CD19^(+)and NK cells between T3 or T4 stage and T1 or T2 stage(P>0.05).The
